Depression is a fundamental disorder characterized by changes in mood or affect toward depression with or without anxiety. Depression is a state of mental disorder characterized by three main symptoms and seven additional symptoms. Diabetes Mellitus Type-1 (T1DM) is a systemic disorder caused by impaired glucose metabolism, which is characterized by chronic hyperglycemia. Depression is common in patients with T1DM. In this case report, a 13-year-old girl was consulted because she looks sad. A feeling of sadness has been felt for 3 months and has been getting worse since a week ago, which was initially because of T1DM. Patients usually work at home and carry out daily activities as usual. Currently, the patient has no desire and gets tired easily, especially when doing usual activities, thinking the pain will get worse and thinking that she can't recover. Complaints are felt throughout the day and worsen at night, especially before bed or when alone. The patient feels that she is useless and his future is bleak. During pregnancy, the patient's mother routinely checks with the midwife and gets complete immunizations. The patient was born vaginally and cried spontaneously. The patient also received complete basic immunization.
Delirium is a syndrome characterized by disturbances of consciousness and cognition that occur acutely and fluctuate. Delirium can be caused by general medical conditions, drug users, sensory disturbances, polypharmacy, etc. The etiology of delirium is diverse and non-specific. The preference of pharmacological therapy in delirium is still a debate. Descriptive research with the retrospective cross-sectional method, using secondary data in medical records at the Sanglah Central General Hospital Denpasar for the period January 1, 2020, to December 31, 2020. Patients who were consulted were 166 people, with sex 57 people (34.3%) and 109 women (65.7%). The incidence of delirium in adults (20-59 years old) and elderly (age 60 years) is the highest with 51.2% and 45.1% respectively. One-third of 45 people (37.2%) had overlapping dementia. Most of the 142 people (85.5%) were experiencing significant life stress. The most common type of delirium was hyperactive delirium (74%). The underlying disease of delirium varies such as the cause of infection (24.1%), intracranial process (19%), malignancy 18.1%, cardiovascular (10.1%), endocrine disorders,(7.8%), kidney disorders (6%), and others in small percentages.
Internet addiction (IA) has been described as 'excessive or poorly controlled preoccupation, urges, or behaviors regarding computer use and internet access that lead to impairment or distress. Excessive internet user has emerged as a leading cause of behavioral and developmental problems in adolescents. There are several factors that influence the occurrence of internet addiction. The risk factors can be categorized by a social factor, psychiatric factor and biological factor. Some studies have found varied results. Molecular and functional imaging techniques have been increasingly used for the analysis of neurobiological changes and neurochemical correlates of IA. MRI (Magnetic Resonance Imaging) studies demonstrate that structural changes in the frontal cortex are associated with functional abnormalities in Internet-addicted subjects. Recent research has implicated the role of the striatal dopaminergic system in the behavioral maladaptation associated with IA. It provided evidence for a molecular genetic link between serotonergic and dopaminergic neurotransmission and Internet addiction like dopamine polymorphism the DRD2/ANKKI Taq Ia and COMT Val158metpolymorphism and the gene coding for the nicotinic acetylcholine receptor subunit alpha 4 (CHRNA4). As both the genetic and personality traits are known to play a role in other addiction it is likely that same mechanism (biopsychosocial) underlie Internet addiction.
