Com a intensificação e diversificação dos fluxos migratórios internacionais para o Brasil e a necessidade de inserção da população imigrante aos serviços públicos de saúde, novas dinâmicas se apresentam no contexto da Atenção Básica. Tendo como estudo de caso a população boliviana que utiliza os serviços da Unidade Básica de Saúde do Bom Retiro, em São Paulo, buscou-se compreender qual o papel dos agentes comunitários de saúde (ACS) para que o sistema público cumpra seu princípio de universalidade, oferecendo acesso às populações imigrantes e as incluindo. Por meio de entrevistas semiestruturadas, examinadas por meio da análise de conteúdo, pôde-se constatar que os agentes comunitários são essenciais para a garantia de inserção dos imigrantes ao serviço de Atenção Básica, pois são ao mesmo tempo sujeitos da promoção em saúde e agentes do território, construindo redes de vínculos que favorecem o cuidado integral.
Trata-se de uma pesquisa qualitativa que utilizou entrevistas semiestruturadas, realizadas com trinta bolivianos e com 49 profissionais de saúde que os atendem na Unidade Básica de Saúde do Bom Retiro, bairro de São Paulo, Brasil. Essas entrevistas foram analisadas por meio de cinco categorias temáticas, utilizando o método de análise de conteúdo. Os resultados mostraram que a garantia do acesso não é o bastante para que o cuidado em saúde à população imigrante aconteça. Os atores políticos e os sujeitos sociais precisam estar envolvidos, organizados e preparados para que esse serviço realmente ocorra.
IntroductionPublic consultation is a strategy for patient and public involvement (PPI) assured by the National Committee for Health Technology Incorporation in Brazilian Public Health System (CONITEC)´s normative framework. It aims to obtain society’s views on the matters under evaluation. It is made available after CONITEC issues an initial recommendation, which will be reviewed in light of society contributions. This paper examines the cases where the final recommendations differed from the initial ones in technology assessments carried out between 2012 and 2020, taking into account the arguments used by the CONITEC Plenary to support the modification.MethodsFrom a total of 361 public consultations referring to the assessment of health technologies carried out by CONITEC between 2012 and 2020, 40 final reports of health technology assessment (HTA) processes that had changes from the initial to the final recommendation were examined. Then, analytical categories were built so that justifications for the change were investigated and the interpretations produced. The empirical approach excluded public consultations regarding clinical protocols and therapeutic guidelines.ResultsIt was observed that, in 39 cases, the change occurred from an unfavorable position to a favorable position for the technology incorporation. The main reasons for changing the plenary’s decision were proposals to renegotiate the prices of technologies (that generate changes in the results of economic evaluation and budget impact analysis), the receipt of new evidence and experience, and opinion arguments sent mainly by health professionals.ConclusionsThe analyzed data has shown that the changes in recommendations are mostly based on the content of contributions which led to changes in the assessment of clinical evidence, economic evaluation and budget impact. Thus, it appears that public consultation has been consolidated as a relevant instrument to inform the deliberative process in HTA in Brazilian Public Health System.
Objetivo: Caracterizar os participantes que responderam os formulários de experiência e opinião das consultas públicas sobre avaliação de tecnologias em saúde, abertas pela Conitec no período de 2015 a 2021. Métodos: Foi realizada uma análise descritiva quantitativa a partir da compilação e tabulação dos dados sociodemográficos dos participantes das consultas públicas. Resultados: Entre 2015 e 2021, foram abertas 256 consultas públicas para avaliação de medicamentos, 46 de procedimentos e 22 sobre produtos de saúde. Verificou-se que a maior parte dos participantes é composta por familiares, amigos ou cuidadores de pacientes com a doença relacionada à tecnologia em avaliação e que 70% são do sexo feminino. Com relação à idade, 48% dos participantes têm de 25 a 39 anos. A participação por cor/etnia é de 69% de pessoas brancas, 24% pardas, 5% pretas e 2% amarelas. Sobre as Regiões de residência dos participantes, 55% são do Sudeste, 18% do Sul, 17% do Nordeste, 7% Centro-Oeste e 3% do Norte. Conclusão: Observa-se que apesar da criação dos formulários de experiência e opinião, desigualdades no perfil dos participantes ainda existem. Evidenciar esses dados contribui para planejar ações futuras, alinhadas aos princípios de igualdade do SUS e que contribuam para a construção de uma participação plural, no que diz respeito à diversidade etária, racial, de sexo e regional.
IntroductionThe Department of Management and Incorporation of Technologies and Innovation in Health (DGITIS) acts as Conitec's Executive Secretariat. Among its attributions, it promotes the public/patient involvement in the health technology assessment (HTA) process. Recently, Conitec has been working on the inclusion of patient's testimonials about their illness experience in the plenary sessions, that is, the monthly meeting where technologies are assessed.MethodsTo support the action of including patient reporting in Conitec's HTA process, DGITIS developed research on HTA agencies websites worldwide. The main criteria was the inclusion of patients’ reports in their Committee meetings. DGITIS contacted some of these agencies and requested a listserv question to the International Network of Agencies for Health Technology Assessment (INAHTA) members. These findings supported the DGITIS for the inclusion of patient participation in Conitec's meetings, from the selection process to the actual participation.ResultsFor the Conitec's HTA process, the patients’ participation should occur in the prior session to the public consultation, guaranteeing the inclusion of their perspective since the recommendation process beginning. Hence, every demand for incorporation to be discussed at Conitec's meeting should be preceded by a public call for patients with the clinical condition. The DGITIS will also hold preparatory meetings, which will serve as moments for shared construction of knowledge and literacy.ConclusionsThe nomination process, so far, has been grounded as a consensus among the patients. Thus, Conitec acts as a mediator, connecting the involved stakeholders, in a way that they can autonomously organize themselves and indicate the main representative and an alternate one. With the inclusion of the patient's perspective in the Conitec's meeting, another form of patient participation was opened in the HTA process. Therefore, the consolidation of this participation space is feasible and contributes to enrich the Brazilian HTA process.
