A modified Komarov-Marks esophagostomy is described. It was the aim of our experiments to show that this procedure does not harm the vagal innervation of the stomach. The vagal function was assessed by testing (1) basal secretion, (2) 2-deoxy-D-glucose-stimulated secretion, (3) pentagastrin-stimulated secretion, and (4) serum gastrin levels in five mongrel dogs. Each test was performed at least three times in each dog before and after the esophagostomy procedure, respectively. The secretion rates were not affected by surgery. It is concluded that cervical esophagostomy in dogs does not affect vagal stimulation of gastric acid secretion. For the application of tracer substances into the duodenum, two types of conduits are described: (1) a short jejunal segment and (2) a jejunal segment with an invagination valve. The pros and cons of these procedures are discussed.
The poster presents a new documentation system for the capture of data on the approx. 17,000 annual Notarzt cases in Munich. The computer-aided analysis of these data yields pointers with regard to the primary techno-organizational and medical aspects of the Notarzt service, such as a)justification of Notarzt calls in the light of a pre-defined list of indications; b) appropriate application of the technical facilities and drugs carried by the Notarzt ambulances; c) correlation between pre-clinical and final diagnoses; d) consistency of the preclinical measures with both the pre-clinical and the final diagnosis. Typical analyses are also presented.
An established method for the experimental approach to the duodenum in the dog is the interposition of a segment of small bowel. However, the leakage of intestinal fluid causes unpleasant side effects. By the invagination within the intestinal segment as valve can be constructed which provides both continence and easy access.
