BackgroundProteins citrullination contributes to generate anticitrullinated peptide antibodies (ACPA) in rheumatoid arthritis (RA). Porphyromonas gingivalis (Pg) is one of main germs incriminated in the development of periodontitis (PD), it has an enzyme called peptidyl arginine deiminase which is able to citrullinate the host proteinsObjectivesThe aim of this study was to seek for a possible association between ACPA and periodontitisMethodsWe conducted a case-control study of 69 patients with early rheumatoid arthritis (≤2 years), naive of biotherapy and 138 age-and sex matched healthy controls. Smokers, diabetics, and subjects who received dental care and those who used antibiotics in the previous 6 months were not included. Demographic data and ACPA were determined. A periodontal examination was performed to all participants. Subgingival plaque samples were analysed to seek for Porphyromonas gingivalis(Pg) in both population in the case of periodontitis.ResultsThe mean age of our patients was 40.75±12.04, the mean duration of the illness was 14.30±6.76 months (extremes: 1–24 months). ACPA was detected in 88% of patients and the mean titre was 255.57±409.78. PD frequency was higher in patient with PR compared with healthy controls (43% versus 29%) and a significant association was found between PR and PD (p<10–3). Patients with RA had 2.46 (CI 1.12 to 5.39) higher odds of having PD compared with healthy controls. In early RA, ACPA titre and rate was not associated with PD (p=0.06,p=0.44 respectively). Regarding the frequency of Porphyromonas gingivalis, there was no significant difference between the PR group and the control group (p=0.45). In addition, there was no significant difference between RA group and controls (p=0.68) concerning Porphyromonas gingivalis and ACPAConclusionsPeriodontitis is a risk factor for the occurrence of rheumatoid arthritis. The ACPA does not seem to be related to periodontitis. BackgroundThe Periodontitis (PD) is currently recognized as a risk factor of occurrence of the rheumatoid arthritis(RA). The porphyromonas gingivalis (P.gingivalis) figure among the main germs incriminated in the occurrence of the PD. Previous studies have shown that antibodies to P. gingivalis are associated with anti-CCP antibody in patients with RA. However others studies have not demonstrated this association.ObjectivesThe aim of this study was to seek for a possible association between clinico-biological parameters of the RA and the presence of the P.gingivalis.MethodsWe conducted a prospective study of 69 patients with early rheumatoid arthritis (≤2 years), naive of biotherapy. Smokers, diabetics, and subjects who received dental care and those who used antibiotics in the previous 6 months were not included. Periodontal status, demographic, clinical activity, and anticyclic citrullinated peptide antibodies (anti-CCP) parameters were determined. The P. gingivalis has been searched in patients with a PD. We sought to investigate an association between Rheumatoid disease activity (DAS28), anticyclic citrullinated peptide antibodies (anti-CCP) and P.gingivalis.ResultsThe mean age of our patients was 40.75 ± 12.04, the mean duration of the illness was 14.30±6.76 months (extremes: 1-24 months). ACPA was detected in 88% of patients and the mean titre was 255.57±409.78. 43% of patients have presented a PD. The P.gingivalis has been detected in 59% of PD. The means DAS28 of patient with and without P.gingivalis is respectively 4.40±1.32 and 4.15±1.45, and there was no significant difference (p=0.65). There was also no association observed between anti CCP and the presence of P.gingivalis(the mean titre of anti CCP was 249.47±294.58 with P.gingivalis and 258.67±93.48 without P.gingivalis, p= 0.74).ConclusionThis study showed that periodontitis is frequent in rheumatoid arthritis. More than half of our patients suffering of periodontitis were infected by porphyromonas gingivalis. Rheumatoid disease activity does not seem to be related to porphyromonas gingivalis. IntroductionRheumatoid arthritis (RA) and periodontitis (PD) are two chronic inflammatory affections that share many similarities. Many studies report the increased frequency of periodontitis during rheumatoid arthritis. The porphyromonas gingivalis, periodontopathogenic bacterium plays a main role through peptidylarginine-deiminase an enzyme capable of citrullinating endogenous proteins and generates an autoimmune reaction.ObjectivesThe aim of this study was to determine the prevalence of periodontitis during rheumatoid arthritis and its relationship with RA severity, treatment and patient characteristics.MethodsThis is a multicentre,cross-sectional, prospective study. Were included adults RA. We identified sociodemographic factors, the periodontal disease status and anaerobic germs, clinical activity of RA with the Disease activity score (DAS 28-VS), the Health Assessment Questionnaire (HAQ), extraarticular manifestations, Sharp Score and periodontal anaerobic bacteria. Were determined correlations between PD and patient characteristics, treatment and RA severity.Resultsa total of 204 RA were included. The mean age of the patients was 50.338±12.428 years. Sex-ratio: 0.13. The mean duration disease was 10.259±8.232 years. 43% of patients had extaarticular manifestations: 43%. The DAS vs-28 =4.615±1.440. HAQ =1.140±0.821.0. The ACPA rate was 229.855±241.902. Sharp mean: 112.995±93.965. 65.7% patients were treated by methotrexate. 32.8% by biological. 77, 4% patients were under corticoids. The periodontitis frequency was 37.3% (51.1% related to the porphyromonas gingivalis (PG). The tooth loss frequency was 31, 4%.the comparison of the subjects with periodontitis and patients without it showed no significant differences. the comparison of the patients with severe periodontitis and those without showed: age p=0.0004, menopause p=0.002, DAS p 0.47, Sharp p=0.06, HAQ p=0.95.ConclusionsThese results suggest a high prevalence of periodontitis during RA, PG is often responsible. The tooth loss frequency is also high. There is no correlation between periodontitis and RA severity and no relationship between PD and RA treatment.
