The above study was conducted to find out the perinatal outcomes in pregnancies with oligohydramnios, especially in the third trimester of pregnancy. METHODS: This prospective and observational study was conducted in a Medical college and research centre at Moradabad city, U. P, from January to December 2012. In this study 78 singleton pregnant women with gestational age from 28-42 weeks with less amniotic fluid index (AFI) were analyzed for perinatal outcome. Data were expressed as number (percentage). Proportion test was performed for comparison between two groups, P value <0. 05 was taken as level of significance. RESULTS: Women with oligohydramnios were significantly associated with an abnormal antepartum fetal heart rate (FHR), meconium stained fluid, Apgar score less than 7 or NICU admission. Also subjects with AFI of 5. 0 cm or less had a higher rate of cesarean section for fetal distress. CONCLUSIONS: Antepartum oligohydramnios is associated with an increased risk of fetal heart rate abnormalities. Although in our population it is not predictive of adverse perinatal outcome as measured by low Apgar score and NICU admission, yet this may be reflective of the aggressive antepartum and intrapartum management that these patients received.
Objective: To compare the effectiveness and tolerability of misoprostol as a cervical ripening agent in first trimester abortion through sublingual and vaginal routes of administration.Material and Methods: This study was carried out in the department of Obst. & Gynae Teerthanker Mahaveer Medical College and Research Centre (TMMC&RC). A total of 120 patients were included in the study. They were divided in two groups Group A 60 patients sublingual Group B 60 patients vaginal The drug was administered 3-4 hours before suction and evacuation by sublingual and vaginal routes. Efficacy was assessed on the basis of time taken for ripening, dilatation achieved, duration of the procedure, intraoperative blood loss and pain. The patients tolerability was noted on the basis of side effects.Results: The mean time taken for cervical ripening was more in the sublingual group as compared to the vaginal ( P<0.001).The duration of suction and evacuation was less as compared to the vaginal route. The mean intraoperative blood loss was more in sublingual as compared to the vaginal group. The intraoperative pain score was comparatively lower (P<0.05) as compared to the vaginal route. Side effects like loose motions, nausea vomiting were more with sublingual group.Conclusion: Sublingual Misoprostol is an effective and favourable cervical ripening agent for 1st trimester abortions.Bangladesh J Obstet Gynaecol, 2012; Vol. 27(2) : 63-66
Developmental anomalies involving Mullerian ducts are one of the most fascinating disorders in Gynaecology. The incidence rates vary widely and have been described between 0.1-3.5% in the general population. We report a case of a fifteen year old girl who presented with primary amenorrhea and lower abdomen pain, with history of instrumentation about two months back. She was found to have abdominal lump of sixteen weeks size uterus. On examination vagina was found to be represented as a small blind pouch measuring 2-3cms in length. A rectovaginal fistula (2x2 cms) was also observed. Ultrasonography of abdomen revealed bulky uterus (size 11.2x6 cm) with 150 millilitre of collection. Adiagnosis of hematometra with iatrogenic fistula was made. Vaginal drainage of hematometra was done which was followed by laparotomy. Peroperatively she was found to have a left side unicornuate uterus with right side small rudimentary horn. Left fallopian tube and ovary showed dense adhesions and multiple endometriotic implants. Both cervix and vagina were absent. Total abdominal hysterectomy was done and rectovaginal fistula repaired. The present case is reported due to its rarity as it involved both mullerian agenesis with cervical and vaginal agenesis along with disorder of lateral fusion. This is an asymmetric type of mullerian duct development in which arrest has occurred in different stages of development on two sides.
