Conclusões: O farmacêutico clínico é de fundamental importância no ambiente hospitalar, visto que o estudo revelou-se uma frequência de 25,66% de interações medicamentosas e 2,26% de incompatibilidades.Realizou-se ainda, intervenções farmacêuticas de trocas de horários de administração de medicamentos em 5,66% das prescrições e em 4,52% de reconciliação medicamentosa, ressaltando assim, que o Serviço de Farmácia Clínica tem grande impacto no aumento da segurança ao paciente e prevenção de eventos adversos. Background and Objectives:The practice of Clinical Pharmacy in hospitals is of utmost importance, as the pharmacist cares for the patient, making it possible the pharmacotherapy optimization, promoting the health and well-being, and preventing diseases and improving the quality of health care. Therefore, the aim of this study was to quantify the occurrence of potential drug interactions, incompatibilities and pharmaceutical interventions carried out on prescriptions of patients treated at an Emergency Care Unit (ECU). Methods: This is a descriptive and quantitative study. The activities were developed in a teaching hospital located in the interior of the state of Rio Grande do Sul from April to July 2016, by resident pharmacists. The study sample consisted of 265 prescriptions of 107 patients. Results: There was a predominance of males (51.4%), with a mean age of 63.41 years. 2,087 prescribed drugs were identified, with a mean of 7.9 (± 3.4) drugs per prescription. Of the 265 analyzed prescriptions, drug interactions were found in 25.6%, as well as 2.3% of drug incompatibilities. RESUMO ABSTRACT
