OBJECTIVE:To estimate the prevalence of dental visits among preschool children and determine the factors associated with using dental services. METHODS:A cross-sectional study was conducted with 1,129 fi ve-year-old children from the Pelotas Birth Cohort Study in Pelotas (Southern Brazil) 2004, from September 2009 to January 2010. Use of dental services at least once in the child's life and the reason for the child's fi rst dental visit were recorded. The categories assigned for the fi rst dental visit were: routine checkup, resolution of a problem, or never saw a dentist. The oral examinations and interviews were performed in the children's homes. Socioeconomic aspects and independent variables related to the mother and child were analyzed using multivariable logistic regression. RESULTS:The prevalence of dental visits (both categories combined) was 37.0%. The main predictors for a routine visit were higher economic status, mothers with more schooling, and mothers who had received guidance about prevention. Major predictors for a visit because of a problem were having felt pain in the previous six months, mothers with higher education level, and mothers who had received guidance about prevention. Approximately 45.0% of mothers received information about how to prevent cavities, usually from the dentist. Children of mothers who adhered to health programs were more likely to have had a routine dental visit. CONCLUSIONS:The rate of preschool visits to dental services was lower than the rate for medical appointments (childcare). In addition to income and education, maternal behavior plays an important role in routine visits. Pain reported in the last six months and a high number of teeth affected by tooth decay, independent of other factors, were associated with visits for a specifi c problem. It is important to integrate oral health instruction into maternal and child health programs.
OBJECTIVE:To estimate the prevalence of regular use of dental care services by adults and older adults residing in vulnerable community and to identify associated factors. METHODS:A population-based cross-sectional study was carried out with 3,391 adults and older adults residing in areas of social vulnerability in Porto Alegre, Southern Brazil, from July to December of 2009. A systematic sampling method was used the selection probability proportional to the population of each of the the 121 census sectors. The outcome for regular use of dental care services was defi ned as regular use of dental services, regardless of the presence of dental problems. A standardized questionnaire was administered, which included demographic, socioeconomic, type of dental care services, selfperception of dental health and self-perceived needs variables. A chi-square test for heterogeneity was used for bivariate analyses, and a Poisson regression with a robust variance and Wald tests were performed for the adjusted analysis. RESULTS:The prevalence of regular use of dental services was 25.7%. The prevalence was higher among people with ≥12 years schooling (PR 2.48 [95%CI:1.96;3.15]), higher income (PR 1.95[95%CI: 1.03;1.53]), use of private health services (PR 1.43 [95%CI: 1.20;1.71]),excellent self-perceived oral health (PR 4.44 [95%CI: 3.07;6.42]) and a self-perceived need for consultation related to routine checkup (RP 2.13 [95%CI: 1,54;2,96]). CONCLUSIONS:Inequalities were found in the regular use of dental services. Integrated approaches that raise awareness of oral health, improve self-care and expand access to dental services, may contribute to increase the use of dental services on a regular basis.
Resumo Objetiva-se verificar a associação entre o uso regular de serviços odontológicos e a perda dentária por idosos vinculados a onze Unidades de Saúde da Família no sul do Brasil. Estudo transversal que avaliou 438 idosos. Um questionário padronizado foi utilizado e as variáveis clínicas de saúde bucal foram obtidas por um dentista treinado. O relato do uso regular dos serviços odontológicos, desfecho do estudo, foi obtido por meio de pergunta única. Foram realizadas análises descritivas e regressão de Poisson com o Stata 12.0. Analisando as variáveis de exposição e desfecho do estudo, na regressão não ajustada, houve associação positiva do relato do uso regular dos serviços de saúde bucal dos idosos com 9-11 anos de estudo (RP = 3,89; IC95%1,77-8,58) em comparação aos idosos com menos de 4 anos de estudo, com até 9 dentes (RP = 2,50;IC95%19,0-5,72) e 10 ou mais dentes (RP = 3,89;IC95%1,58-9,57) em comparação aos idosos sem dentes. Ao considerar a exposição principal, perda dentária, na análise ajustada, os indivíduos com 10 ou mais dentes (RP = 3,51;IC95%1,37-8,99) apresentavam maiores prevalências de relato de uso regular em comparação aos indivíduos sem dentes. O estudo identificou que ter dentes está associado positivamente ao relato do uso regular dos serviços de saúde bucal entre os idosos.
