BackgroundAssessing the clinical effectiveness of measuring grip strength as a prognostic tool in recovering ambulation in bed-confined frail elderly patients.MethodsA prospective study was carried out with 50 elderly inpatients (mean age: 81.6 years old). Manual muscle test was used for checking strength of hip flexor muscles, hip abductor muscles and knee extensor muscles. Grip strength was assessed by hydraulic dynamometer. Walking ability was assessed by functional ambulation categories and Functional Classification of Sagunto Hospital Ambulation. Existence of cognitive impairment (Short Portable Mental Status of Pfeiffer) and comorbidity (abbreviated Charlson index) were considered to be confounding variables.Statistical analysisSimple comparisons and mixed models of multiple ordinal regression.ResultsThe sample presented generalized weakness in scapular (mean 4.22) and pelvic (mean 3.82) muscle. Mean hand grip values were similar: 11.98 kg right hand; 11.70 kg left hand. The patients had lost walking ability. After treatment, there was a statistically significant for scapular waist strength (P=0.001), pelvic waist strength (P=0.005) and walking ability (P=0.001). A statistically significant relationship in the regression analysis was found between the grip (right and left hands) and walking ability post-treatment (P=0.009; odds ratio 1.14 and P=0.0014 odds ratio 1.113 for each walking scale). The confounding variables showed no statistical significance in the results.ConclusionGrip strength is associated with walking ability in hospitalized frail elderly. Grip strength assessment by hydraulic dynamometry is useful in patients with poor collaboration. Walking ability training in frail elderly inpatients is useful.
BackgroundThe health-related quality of life in stroke patients (HRQOLISP-40, short version) survey was developed in Nigeria and constitutes a 40-item, multidimensional, self-administrated questionnaire. We assessed the validity and reliability of the HRQOLISP-40 Spanish version for stroke patients in Colombia.MethodsThe analysis included factor analysis, confirmatory factor analysis, Rasch analysis, convergent validity, internal consistency (261 stroke patients), test-retest reliability (73 patients assessed at two different times) and sensitivity to change (46 patients assessed before and after a rehabilitation intervention).ResultsWe found an 8-domain structure. None of the items had a significant impact on the global alpha value in order to be removed. Lin’s concordance correlation coefficient indicated test-retest reliability (Rho IC: 0.76 to 0.95), suggesting an adequate stability of the instrument. Regarding sensitivity to change differences, they were only significant in the psychological and eco-social domains (p <0.05). When comparing SF-36 with HRQOLISP-40, all the correlation coefficients values were significantly different from zero, except those related to vitality. The highest scores were found in the physical and physical functioning domains, with a value of 0.722.ConclusionsThe HRQOLISP-40 scale is valid and reliable for assessing patients’ quality of life after a stroke. Validating quality of life assessment instruments is necessary in order to improve the effectiveness of rehabilitation programs for Colombian stroke patients.Electronic supplementary materialThe online version of this article (doi:10.1186/s12883-016-0770-5) contains supplementary material, which is available to authorized users.
La infección por coronavirus Sars-CoV-2 desde su aparición en 2019, se ha manifestado con el compromiso multiorgánico y con la consecuente necesidad de un abordaje multidisciplinario que incluye a varios profesionales de la salud para la intervención aguda, la estabilización del paciente, la exploración neurológica y el proceso de rehabilitación. En conjunto las manifestaciones neurológicas, respiratorias y cardiovasculares son causa de discapacidad, alteración en la funcionalidad, de la participación y del entorno social en los pacientes con infección por Sars-CoV-2, por lo que requieren de una rehabilitación temprana como también de programas diseñados en torno a la pandemia y al compromiso neurológico, con el fin de prevenir complicaciones tempranas. Estas son razones por las que la presente revisión narrativa tiene como fin describir los principales compromisos del sistema nervioso central así como las secuelas neurológicas, basados en la búsqueda y referencia de diferentes tipos de artículos publicados hasta la fecha.
Desde su notificación a finales del 2019, la infección secundaria a coronavirus se ha difundido a nivel global hasta sercalificada como una pandemia. La respuesta inflamatoria generalizada que produce la infección por Sars-CoV-2, compromete múltiples órganos y sistemas, siendo el sistema respiratorio uno de los más afectados y con desenlace fatal, sin embargo, la afectación del sistema nervioso periférico puede implicar el desarrollo de déficits permanentes,con limitaciones en las actividades de la vida diaria yrestricción en la participación, llevando a la pérdida de independencia yreducción de la calidad de vida. El médico rehabilitador debe estar enfocado al reconocimiento y detección temprana de la patología nerviosa periférica, así como a determinar la intervención de la cual se beneficia más el paciente, liderar el equipo interdisciplinario de salud y mantener un trabajo fluido con otras especialidades médicas.
