This is a qualitative research whose objective was to identify the patients' of a Hospital of Custody and Psychiatric Treatment (HCPT) located in Florianópolis, view on the institution. The data were collected in 2004, through semistructured interviews and participant observation. They were analyzed by establishing a parallel between the studies of Goffman on total institution and the norms recommended by the Brazilian Psychiatric Reform. Most of the interviewees referred to be HCPT as a cross between hospital and prison, predominantly of a custodial character. The treatments and the operational conditions of the institution were also questioned by the patients. The conclusion is that the institutional conditions reveal to be way different to the ones considered ideal by the Psychiatric Reform, a situation which demands the adaption of such conditions to the recommendations of the new paradigm on patient care to the mentally-ill. Abstract ResumenEn el presente artículo se desarrolla una investigación cualitativa cuyo objetivo principal fue el de identificar la percepción que los internos del Hospital de Custodia y Tratamiento Psiquiátrico (HCTP) de Florianópolis, tienen con relación a la institución. Los datos fueron recolectados en 2004, a través de entrevistas parcialmente estructuradas y de la observación participante. Estos datos fueron analizados estableciendo un paralelo entre los estudios de Goffman sobre institución total y las normas preconizadas por la Reforma Psiquiátrica Brasileña. Para la mayor par te de los entrevistados, el HCTP es una mezcla de hospital y prisión, predominantemente de custodia. El tratamiento y las condiciones de funcionamiento de la institución también fueron cuestionados por los internos. La conclusión a la que se llegó es de que las condiciones institucionales encontradas en el hospital, se encuentran distantes de las consideradas ideales por la reforma psiquiátrica, siendo necesario su adecuación a lo preconizado por el nuevo paradigma de atención al portador de trastorno mental.Esc Anna Nery R Enferm 2006 dez; 10 (4): 671 -7.
