BackgroundA spontaneous rupture of the unscarred uterus in a primigravid patient is extremely rare and is associated with high perinatal and maternal morbidity and mortality.Case presentationA 34-year-old white primigravid woman, 31 + 3 weeks of gestation, presented with pre-eclampsia and developed a sudden acute abdomen. An emergency laparotomy was performed and a uterine rupture was found as the cause of the event. A stillborn girl was born.ConclusionA rupture of the pregnant uterus should always be considered in a pregnant woman presenting with abdominal pain, even in a primigravid patient.
Oral poster abstracts Results: MCA-PI measurements were available in 122 out of 322 patients with CNS abnormalities. The MCA-PI was normal in 97 patients (79.5%) and in 12 patients (9.8%) it was raised, in seven patients (5.7%) it was initially normal and later increased, and in six patients (4.9%) it was initially increased and later normalized. In patients with spina bifida (n = 39), there were only three patients with an abnormal MCA-PI. In the group with other CNS abnormalities, there was no significant difference in the number of survivors in the group with completely normal MCA-PI measurements (24/59 = 41%) and the group with at least one abnormal MCA-PI 12/24 = 26%) (Chi-square = 0.60, P > 0.05). Amongst the survivors, in the group with completely normal MCA-PI measurements, there were six children with a normal outcome, six with minor neurological morbidity (e.g. aqueductal stenosis treated with a stent) and six with major neurological morbidity (e.g. microcephaly, lissencephaly or corpus callosum agenesis). In the group with al least one abnormal MCA-PI, there were no children with a normal outcome, three with minor neurological morbidity (e.g. aqueductal stenosis treated with a stent) and nine with major neurological morbidity. This difference is not statistically significant (chi-square = 0, 386; P > 0.05). Conclusions: Although not statistically significant, the trend to a poorer outcome in fetuses with a CNS abnormality and abnormal MCA-PI is an interesting finding, suggesting prospective evaluation on a larger scale.
