Mucormycosis is an angioinvasive opportunistic fungal infection, produced by the Mucorales, which occurs mainly in diabetic and immunocompromised patients and can affect any organ or tissue. Within the location of the head and neck, the presentation at the nasal and paranasal sinuses is the most common, the clinical manifestation of mucormycosis at the ear level being exceptional. The figures reveal a high mortality despite antifungal and surgical treatment, due to its aggressive nature and its rapid orbital and cerebral extension. We present a case of a patient diagnosed with malignant otitis externa due to mucor with facial paralysis, being the first manifestation of her diabetes and which evolved favorably after surgery, treatment with amphotericin B and with thorough glycemic control.
