Preeklampsi ön tanısı ile kliniğimizde tetkik ve tedavi edilen gebelerin proteinüri tanısında kullanılan yöntemlerin karşılaştırılması amaçlanmıştır.Gereç ve yöntem: Çalışmaya, Ocak 2016-Aralık 2018 tarihleri arasında bir üniversite hastanesinde preeklampsi ön tanısı ile spot idrarda dipstik yöntemi, P/K oranı ve 24 saatlik idrarda toplam protein analizi yapılan 317 hasta çalışmaya dahil edildi. Hastaların bu üç değeri birbiri ile mukayese edildi.Bulgular: 24 saatlik idrarda toplam proteinüri eşik değeri 300 mg/gün olduğunda P/K oranı cut-off değeri 0,34 mg protein/mg kreatinin, sensitivite %68,9, spesifite %78,7 olarak hesaplandı. Spot idrar P/K oranı ile 24 saatlik idrardaki toplam proteinüri değerleri arasında orta düzeyde kuvvetli, istatistiksel olarak anlamlı korelasyon saptandı (r= 0,629 p<0,001).Sonuç: 24 saatlik toplam idrar analizi için yeterli zamanın olmadığı acil durumlarda spot idrarda P/K oranının 24 saatlik idrarda toplam protein testine alternatif test olarak kullanılabilir.
Objective To investigate the effect of closure types of the anterior abdominal wall layers in cesarean section (CS) surgery on early postoperative findings.
Methods The present study was designed as a prospective cross-sectional study and was conducted at a university hospital between October 2018 and February 2019. A total of 180 patients who underwent CS for various reasons were enrolled in the study. Each patient was randomly assigned to one of three groups: Both parietal peritoneum and rectus abdominis muscle left open (group 1), parietal peritoneum closure only (group 2), and closure of the parietal peritoneum and reapproximation of rectus muscle (group 3). All patients were compared in terms of postoperative pain scores (while lying down and during mobilization), analgesia requirement, and return of bowel motility.
Results The postoperative pain scores were similar at the 2nd, 6th, 12th, and 18th hours while lying down. During mobilization, the postoperative pain scores at 6 and 12 hours were significantly higher in group 2 than in group 3. Diclofenac use was significantly higher in patients in group 1 than in those in group 2. Meperidine requirements were similar among the groups. There was no difference between the groups' first flatus and stool passage times.
Conclusion In the group with only parietal peritoneum closure, the pain scores at the 6th and 12th hours were higher. Rectus abdominis muscle reapproximations were found not to increase the pain score. The closure of the anterior abdominal wall had no effect on the return of bowel motility.
Abstract:Objectıves: Toxoplasmosis is a widely-distributed zoonosis that is caused by Toxoplasma gondii protozoa. Toxoplasmosis is a high prevalence of unapparent infections, and when infected for the first time during pregnancy, can present a temporary parasitemy with focal lesions generated within the placenta, thereby infecting the fetus. Our aim in this study is to evaluate the patients who have been diagnosed with new toxoplasm in our hospital for six months. Methods: In our study, the toxoplasma results of the first trimester pregnant patients who applied to our polyclinic in the second half of 2016 were evaluated. A total of 391 cases were included in the study. Patients whose laboratory results were not available or under-investigated were excluded from the study. In our policlinic, toxoplasmosis IgM is primarily required in first trimester pregnancies. If positive, IgG and avidity are desired. Further investigation is carried out according to the avidity result. SPSS 22 and Microsoft Excel 2007 program were used in the analysis of our work. Results: A total of 8 (2%) patients had positive toxoplasmosis IgM, 4 (1%) patients had high avidity, 1 (0.2%) patients had moderate avidity, and 3 (0.7%) patients had low avidity. Conclusions: There is no consensus on the screening of all pregnancies. Screening of all pregnancies is recommended in places where the general view seroprevalence is high.
