Isolated physeal fractures in the pediatric distal ulna are relatively uncommon. While the skeletally immature patient generally has higher remodeling potential than adults, special considerations should be given to the pediatric wrist. We report the case of a 15-year-old male with an isolated Salter-Harris (SH) type IV distal ulna fracture.
