Human infection by leptospires has highly variable clinical manifestations, which range from subclinical infection to fulminant disease. We conducted a population-based, cross-sectional seroepidemiologic study in Peru to determine potential relationships of environmental context to human exposure to Leptospira and disease associated with seroconversion. Three areas were studied: a flooded, urban slum in the Peruvian Amazon city of Iquitos; rural, peri-Iquitos villages; and a desert shantytown near Lima. Seroprevalence in Belen was 28% (182/650); in rural areas, 17% (52/316); and in a desert shantytown, 0.7% (1/150). Leptospira-infected peridomestic rats were found in all locales. In Belen, 20 (12.4%) of 161 patients seroconverted between dry and wet seasons (an incidence rate of 288/1,000). Seroconversion was associated with history of febrile illness; severe leptospirosis was not seen. Human exposure to Leptospira in the Iquitos region is high, likely related both to the ubiquity of leptospires in the environment and human behavior conducive to transmission from infected zoonotic sources.
Kerosene-fueled wick lamps used in millions of developing-country households are a significant but overlooked source of black carbon (BC) emissions. We present new laboratory and field measurements showing that 7–9% of kerosene consumed by widely used simple wick lamps is converted to carbonaceous particulate matter that is nearly pure BC. These high emission factors increase previous BC emission estimates from kerosene by 20-fold, to 270 Gg/year (90% uncertainty bounds: 110, 590 Gg/year). Aerosol climate forcing on atmosphere and snow from this source is estimated at 22 mW/m2 (8, 48 mW/m2), or 7% of BC forcing by all other energy-related sources. Kerosene lamps have affordable alternatives that pose few clear adoption barriers and would provide immediate benefit to user welfare. The net effect on climate is definitively positive forcing as co-emitted organic carbon is low. No other major BC source has such readily available alternatives, definitive climate forcing effects, and co-benefits. Replacement of kerosene-fueled wick lamps deserves strong consideration for programs that target short-lived climate forcers.
BackgroundDisplacing the use of polluting and inefficient cookstoves in developing countries is necessary to achieve the potential health and environmental benefits sought through clean cooking solutions. Yet little quantitative context has been provided on how much displacement of traditional technologies is needed to achieve targets for household air pollutant concentrations or fuel savings.ObjectivesThis paper provides instructive guidance on the usage of cooking technologies required to achieve health and environmental improvements.MethodsWe evaluated different scenarios of displacement of traditional stoves with use of higher performing technologies. The air quality and fuel consumption impacts were estimated for these scenarios using a single-zone box model of indoor air quality and ratios of thermal efficiency.ResultsStove performance and usage should be considered together, as lower performing stoves can result in similar or greater benefits than a higher performing stove if the lower performing stove has considerably higher displacement of the baseline stove. Based on the indoor air quality model, there are multiple performance–usage scenarios for achieving modest indoor air quality improvements. To meet World Health Organization guidance levels, however, three-stone fire and basic charcoal stove usage must be nearly eliminated to achieve the particulate matter target (< 1–3 hr/week), and substantially limited to meet the carbon monoxide guideline (< 7–9 hr/week).ConclusionsModerate health gains may be achieved with various performance–usage scenarios. The greatest benefits are estimated to be achieved by near-complete displacement of traditional stoves with clean technologies, emphasizing the need to shift in the long term to near exclusive use of clean fuels and stoves. The performance–usage scenarios are also provided as a tool to guide technology selection and prioritize behavior change opportunities to maximize impact.CitationJohnson MA, Chiang RA. 2015.
