Objective To evaluate the efficacy of a brief cognitive-behavioral therapy (CBT) that is being developed for management of cognitive dysfunction following chemotherapy among breast cancer survivors. Memory and Attention Adaptation Training (MAAT) is a brief CBT designed to improve the quality of life and function among cancer survivors with post-chemotherapy cognitive complaints. Methods An initial, two-group (MAAT versus waitlist, no treatment control), randomized clinical trial (RCT) was conducted. Forty stage I and II female breast cancer survivors (mean age = 50; SD = 6.4) were randomized to conditions and assessed at baseline, post-treatment (8 weeks) and 2-month follow-up assessment points on measures of: (1) self-reported daily cognitive failures; (2) quality of life; and (3) neuropsychological performance. Participants were also assessed for satisfaction with MAAT. Results With education and IQ as covariates, MAAT participants made significant improvements relative to controls on the spiritual well-being subscale of the quality of life measure and on verbal memory, but statistical significance was not achieved on self-report of daily cognitive complaints. However, moderate-to-large effect sizes were observed on these outcomes. Participants gave MAAT high satisfaction ratings. Conclusions Although this initial RCT is a small study, MAAT participants appear to improve on one measure of quality of life and verbal memory performance relative to no treatment controls and rate MAAT with high satisfaction. These data are encouraging and support the continued development and evaluation of MAAT efficacy.
Research Summary The relationship between reliability and validity is an important but often overlooked topic of research on risk assessment tools in the criminal justice system. By using data from the Minnesota Screening Tool Assessing Recidivism Risk (MnSTARR), a risk assessment instrument the Minnesota Department of Corrections (MnDOC) developed and began using in 2013, we evaluated the impact of inter‐rater reliability (IRR) on predictive performance (validity) among offenders released in 2014. After comparing the reliability of a manual scoring process with an automated one, we found the MnSTARR was scored with a high degree of consistency by MnDOC staff as intraclass correlation (ICC) values ranged from 0.81 to 0.94. But despite this level of IRR, we still observed a degradation in predictive validity given that automated assessments significantly outperformed those that had been scored manually. Additional analyses revealed that the more inter‐rater disagreement increased, the more predictive performance decreased. The results from our cost–benefit analyses, which examined the anticipated impact of the MnDOC's efforts to automate the MnSTARR, showed that for every dollar to be spent on automation, the estimated return will be at least $4.35 within the first year and as much as $21.74 after the fifth year. Policy Implications Although it is unclear the degree to which our findings, which are somewhat preliminary, are generalizable to other offender populations and correctional systems, we believe the results are sufficiently promising to warrant greater interest in automating the assessment of risk and need. We anticipate many, if not most, correctional systems may need to invest in upgrading their IT infrastructure to support the use of automated instruments. But we also anticipate this investment would deliver a favorable return for our results suggest that automation reduces inter‐rater disagreement, which in turn improves predictive performance. Even if automation did not improve performance, the increased efficiency it produces would create reinvestment opportunities within correctional systems.
Research on desistance or the process of ceasing criminal behavior has increased in recent years. This work has revealed a large number of factors that are related to desistance. To date, these explanations have been generally perceived as mutually exclusive and/or competitive. Interestingly, while research on desistance from crime has been a recent focus, certain work had examined crime over the life-course as far back as the early 20th century. In particular, the Gluecks offered one of the earliest "theories" of desistance, focusing on maturation. Their "maturation theory" was somewhat tautological and not well specified. However, the Gluecks were clear that further work was needed in order to specify what maturation means and how it relates to crime. In this article, five domains of maturation are articulated drawing on the literature in the life-course and developmental fields. It is argued that this new framework may help advance the criminological work on desistance.
School dropout has been extensively studied in the literature as a correlate of negative life outcomes. A precursor to school dropout is truancy, the unexcused or illegitimate student absence from school. Few studies have examined the relationship between truancy and involvement in crime and adjustment more generally over the life-course. This study extends previous work by exploring whether truancy at age 12 to 14 is related to later life outcomes such as crime, aggression, and adjustment using data from the Cambridge Study in Delinquent Development. Results indicate that truancy has long-lasting associations with negative life outcomes, especially for non-violent crime and problem drinking. Importantly, these findings hold for certain outcomes controlling for a comprehensive host of environmental and individual childhood risk factors.
