ObjectiveTo establish the prevalence of physical, cognitive and psychiatric disabilities, associated factors and their relationship with the qualities of life of intensive care survivors in Brazil.MethodsA prospective multicenter cohort study is currently being conducted at 10 adult medical-surgical intensive care units representative of the 5 Brazilian geopolitical regions. Patients aged ≥ 18 years who are discharged from the participating intensive care units and stay 72 hours or more in the intensive care unit for medical or emergency surgery admissions or 120 hours or more for elective surgery admissions are consecutively included. Patients are followed up for a period of one year by means of structured telephone interviews conducted at 3, 6 and 12 months after discharge from the intensive care unit. The outcomes are functional dependence, cognitive dysfunction, anxiety and depression symptoms, posttraumatic stress symptoms, health-related quality of life, rehospitalization and long-term mortality.DiscussionThe present study has the potential to contribute to current knowledge of the prevalence and factors associated with postintensive care syndrome among adult intensive care survivors in Brazil. In addition, an association might be established between postintensive care syndrome and health-related quality of life.
RESUMODescrevem-se os aspectos clínicos da dilatação cística do úraco e uroperitônio em cinco touros. Os animais apresentaram, em datas distintas, distensão abdominal e diminuição da ingestão de alimentos e água, até culminar com inapetência, cerca de duas semanas após o aparecimento dos primeiros sintomas. Ocorreu distensão abdominal bilateral progressiva, que, no início do processo, era discreta e restrita ao quadrante inferior do abdômen; com cerca de duas semanas de evolução, o abdômen assumiu forma arredondada semelhante à pera. Observou-se bruxismo, atonia ruminal e desidratação. A abdominocentese revelou a presença de líquido amarelado com concentração de ureia superior a 200mg/dL. A concentração de ureia no soro sanguíneo variou de 220 a 280mg/dL e a creatinina de 65 a 82mg/dL. A ligadura do divertículo do úraco próximo ao vértex da bexiga foi eficaz nos quatro touros operados. Em partos normais, o cordão umbilical exterioriza-se aproximadamente 10cm fora da parede abdominal, e não existe, nos bovinos, um ponto extra-abdominal de ruptura do cordão, o que faz variar o tamanho do umbigo que fica exposto. Dentre as transformações anatomorfisiológicas que ocorrem a partir da ruptura do cordão umbilical, destaca-se a excreção urinária que era feita pelo úraco e passa a ser realizada pela uretra. ABSTRACT. Differences between reactive oxygen species and antioxidant defense system unbalance the redox status. The exposure to cigarette smoke can increase this imbalance. Trans-resveratrol is a polyphenol with great antioxidant action that reduces the oxidative stress. This study investigated the effect of the trans-resveratrol supplementation on the cardiac oxidative stress in rats exposed to cigarette smoke. Male Wistar rats were randomized into four groups: Control Group (CG), Exposure to Smoke Group (ESG), Antioxidant Group (AG) and Exposure to Smoke plus Antioxidant Group (ESAG). Animals were exposed to cigarette smoke and supplemented with trans-resveratrol (6.0 mg kg -1 ) for two months. The lipid peroxidation (TBARS) and the enzymatic activity of catalase (CAT) were measured in the cardiac muscle. The ESG presented the highest lipid peroxidation level compared with CG (p < 0.001), AG (p < 0.001) and ESAG (p < 0.006). The CAT activity was higher in the AG (p < 0.001) and ESAG (p < 0.001) compared with CG. The ESG presented lower CAT activity compared with the ESAG (p < 0.001). The supplementation of Trans-resveratrol attenuated the cardiac oxidative stress and increased the activity of catalase. Diferenças entre espécies reativas de oxigênio e sistema de defesa antioxidante desequilibram o estado redox. Exposição à fumaça de cigarro pode aumentar esse desequilíbrio. Trans-resveratrol é um polifenol com ação antioxidante que reduz o estresse oxidativo. O objetivo do presente estudo foi investigar os efeitos da suplementação com trans-resveratrol no estresse oxidativo cardíaco de ratos expostos à fumaça de cigarro. Randomização de 32 ratos Wistar machos em quatro grupos: Controle (CG), Exposição à Fumaça (ESG), Antioxidante (AG) e Exposição à Fumaça+Antioxidante (ESAG). Animais foram expostos à fumaça de cigarro e suplementados trans-resveratrol (6,0 mg kg -1 ) durante dois meses. Lipoperoxidação (TBARS) e atividade enzimática da catalase (CAT) foram mensuradas no músculo cardíaco. ESG apresentou maiores níveis de lipoperoxidação quando comparado ao CG (p < 0,001), AG (p < 0,001) e ao ESAG (p < 0,006). Atividade da CAT foi maior no AG (p < 0,001) e no ESAG (p < 0,001) quando comparados ao CG. ESG apresentou a menor atividade da CAT quando comparado ao ESAG (p < 0,001). A suplementação com trans-resveratrol atenuou o estresse oxidativo cardíaco e aumentou a atividade enzimática de defesa catalase. Background. Respiratory monitoring of mechanical ventilation (MV) is relevant and challenging in COVID-19. Mechanical power (MP) is a novel and promising monitoring tool in acute distress respiratory syndrome (ARDS), representing the amount of energy transferred from the ventilator to the patient. It encompasses several setting parameters and patient-dependent variables that could cause lung injury. MP can therefore be an additional tool in the assessment of these patients. Objective. This study aims to evaluate respiratory monitoring through MP and its relationship with mortality in patients with COVID-19-related ARDS (CARDS) under mechanical ventilation (MV) and prone position (PP) strategies. Methods. Retrospective, unicentric, and cohort studies. We included patients with CARDS under invasive MV and PP strategies. Information regarding MP, ventilation, and gas exchange was collected at 3 moments: (1) prior to the first PP, (2) during the first PP, and (3) during the last PP. We tested the relationship between MP and VR with in-hospital mortality. Results. We included 91 patients. There was a statistically significant difference in MP measurements between survivors and nonsurvivors only in the last prone position ( p < 0.001 ). This is due to the significant increase in MP measurements in nonsurvivors (difference from the baseline: 3.63 J/min; 95% CI: 0.31 to 6.94), which was not observed in the group that survived (difference from the baseline: 0.02 J/min; 95% CI: −2.66 to 2.70). In multivariate analysis, MP ( p = 0.009 ) was associated with hospital death when corrected for confounder variables (SAPS 3 score, mechanical ventilation time, age, and number of prone sessions). Conclusions. MP is an independent predictor of mortality in PP patients with CARDS.
