nurses who were 30 yr old or more. WHR in female nurses increased slightly according to increasing duration of shift work. Fasting blood sugar was not significantly associated with the duration of shift work in either sex regardless of age-group. These results suggest an association between shift work duration and the metabolic risk factors of cardiovascular disease. The number of shift workers has increased in some branches of industry, especially manufacturing. The total number of night shift and other types of shift workers ranges from 15 to 20% of the total workforce in most European Community countries 1) , and is 20% in the United States 2) . In Asian countries, the numbers of shift workers are gradually increasing, and it has been estimated that about 6% of the total work force in Thailand, 10% in the Philippines, and 14% in Peru were involved with shift work during the 1980's 3) . Nationwide surveys showed the percentages of shift workers were 32% in Singapore (1991) 4) and 25% in Korea (1994) 5) . There have been several reports on the incidence of cardiovascular diseases (CVD) among shift workers. Most of these studies found significant associations between CVD and shift work 1,6) . The longer the shift work duration, the higher the risk of CVD 7) . The population etiologic fraction of shift work on cardiovascular disease has been estimated to be approximately 7%, due to the high proportion of shift workers in the workforce 8) . One of the mechanisms proposed for this association is a mismatch of circadian rhythms, through which metabolic disturbances could occur 9) .A recent epidemiologic study reported that obesity, high triglycerides (TG) and low concentrations of high-density lipoprotein (HDL) cholesterol appear to cluster together more often in shift workers than in day workers 9) . Background: There are increasing concerns over adverse effects of prenatal phthalate exposure on the neurodevelopment of infants.Objectives: Our goal was to explore the association between prenatal di(2-ethylhexyl) phthalate and dibutyl phthalate exposure and the Mental and Psychomotor Developmental Indices (MDI and PDI, respectively) of the Bayley Scales of Infant Development at 6 months, as part of the Mothers and Children’s Environmental Health Study.Methods: Between 2006 and 2009, 460 mother–infant pairs from Seoul, Cheonan, and Ulsan, Korea, participated. Prenatal mono(2-ethyl-5-hydroxyhexyl) phthalate (MEHHP), mono(2-ethyl-5-oxohexyl) phthalate (MEOHP), and mono-n-butyl phthalate (MBP) were measured in one urine sample acquired from each mother during the third trimester of pregnancy. Associations with log-transformed creatinine-corrected phthalate concentrations were estimated using linear regression models adjusted for potential confounders.Results: MDI was inversely associated with the natural log concentrations (micrograms per gram creatinine) of MEHHP [β = –0.97; confidence interval (CI), –1.85 to –0.08] and MEOHP (β = –0.95; CI, –1.87 to –0.03), and PDI was inversely associated with MEHHP (β = –1.20; CI, –2.33 to –0.08). In males, MDI was inversely associated with MEHHP (β = –1.46; CI, –2.70 to –0.22), MEOHP (β = –1.57; CI, –2.87 to –0.28), and MBP (β = –0.93; CI, –1.82 to –0.05); PDI was inversely associated with MEHHP (β = –2.36; CI, –3.94 to –0.79), MEOHP (β = –2.05; CI, –3.71 to –0.39), and MBP (β = –1.25; CI, –2.40 to –0.11). No significant linear associations were observed for females.Conclusions: The results suggest that prenatal exposure to phthalates may be inversely associated with the MDI and PDI of infants, particularly males, at 6 months.
BackgroundManganese is an essential trace element and common component of water, soil, and air. Prenatal manganese exposure may affect fetal and infantile neurodevelopment, but reports on in utero manganese exposure and infant neurodevelopment are rare.ObjectiveThis study was conducted to investigate a relationship between maternal blood manganese level and neurodevelopment of infants at 6 months of age.MethodsData were obtained from the Mothers and Children’s Environmental Health (MOCEH) birth cohort study. The study population included 232 pairs of pregnant women and their infants at 6 months of age. Maternal blood manganese was measured at term, just before delivery. Mental and psychomotor development in infancy was assessed at 6 months of age using the Bayley Scales of Infant Development. The relationship between maternal blood manganese level and the mental and psychomotor development indexes (MDI and PDI) was estimated for manganese modeled as a linear and as a categorical variable and using penalized splines for nonlinear modeling.ResultsMean ± SD maternal blood manganese concentration was 22.5 ± 6.5 μg/L. After adjustment for potential confounders, blood manganese was used as a continuous variable in a linear and nonlinear model. Associations between maternal blood manganese and MDI and PDI scores followed an inverted U-shape dose–response curve after adjustment for potential confounders, with lower scores associated with both low and high blood concentrations [MDI: likelihood-ratio test (LRT) p = 0.075, PDI: LRT p = 0.038]. Evaluating the health effects and Biomarker of mercury exposure and establishing diagnosis and treatment standards are very difficult.It can implement that evaluating mercury exposure level for diagnosis by a provocation test uses chelating agent and conducting to appropriate therapy according to the result. but, indications for the therapy of chelating agents with mercury exposure have not yet been fully established.The therapy to symptomatic patients with mercury poisoning is chelating agents, combination therapy with chelating agents, plasma exchange, hemodialysis, plasmapheresis. But the further evaluations are necessary for the effects and side effects with each therapy.
