There were no statistically significant differences in sinonasal complications (e.g. synechiae, anosmia, deformity, and sinusitis) between the two groups. Although the perforation rate (especially for perforations in the middle portion of the septum) was statistically greater in Group 1 than in Group 2, the hyposmia rate was statistically greater in Group 2 than in Group 1.
amaç: Endoskopik endonazal transsfenoidal cerrahi, başta hipofiz adenomları olmak üzere sellar ve parasellar bölgenin diğer tümörleri, rinore cerrahisi ve optik sinir cerrahisi gibi pek çok durumda kullanılmaktadır. Bu çalışmada, kliniğimizde endoskopik endonazal cerrahi uygulanan 100 hastanın sonuçları değerlendirilmiştir. Gereç ve Yöntem: Hastaların 53'ü kadın (%53), 47'si erkek (%47) olup, yaş ortalaması 46.8 (15-77) idi. Toplam 100 hastanın patoloji sonucu en sık 59'u sekretuar hipofiz adenomu 28'i non sekretuar adenom olarak tespit edildi. Ayrıca 5 hasta menengiom, 3 hasta kraniofarengiom, 1 hasta kordoma, 1 hasta nöroendokrin tümör, 1 hasta germinom, 1 hasta Rathke kleft kisti, 1 hasta ossifiye fibrom tanısı almıştır. On iki hastada epistaksis, 11 hastada diabetes insipitus, 15 hastada ise bos sızıntısı görülmüş, majör vasküler yaralanma olmamıştır. İki hastada menenjit gelişmiştir. Tartışma: Transsfenoidal cerrahilerde BOS fistülü görülme oranı % 0.5-15, epistaksis % 0.6, geçici DI oranı % 11.99, kalıcı DI oranı ise % 3.41olarak bildirilmiştir. Remisyon ve total rezeksiyon oranları % 60-80 arasında bildirilmiştir. Sonuç: Serimizde epistaksis oranı literatüre göre yüksek bulunmuştur. Geçici Dİ oranı, remisyon ve total rezeksiyon oranı literatürle uyumlu bulunmuş, kalıcı DI oranı ise düşük bulunmuştur. Serimizde majör vasküler yaralanma olmamıştır. İki hasta menenjit ve komplikasyonları nedeniyle kaybedilmiştir. endoscopic endonasal Transsphenoidal Surgery: a retrospective evaluation of 100 cases Objective: Endoscopic endonasal and transsphenoidal surgery have been commonly used primarily for pituitary adenomas, and also other tumors of sellar and parasellar regions and rhinorrhea, and optic nerve surgeries. In the present study, we evaluated results of 100 patients who underwent endoscopic endonasal surgery in our clinic. Material and Methods: Fifty-three female (53%) and 47 male (47%) patients with a median age of 46.8 (15-77) years were included in the study. The most frequently detected pathologies were secretory pituatory adenoma in 59, and non-functional adenoma pituitary adenoma in 28 patients. Other established diagnoses were meningioma (n= 5), craniopharyngioma (n=3), chordoma (n=1), neuroendocrine tumor (n=1), germinoma (n=1), Rathke cleft cyst (n=1), and ossified fibroma (n=1). Epistaxis (n=12), diabetes insipidus (n= 11), CSF leak ( n= 15) in respective number of patients. Major vascular injuries did not occur. Two patients developed menengitis. Discussion: After transsphenoidal surgeries CSF fistula (0.5-15%), epistaxis (0.6%), transient DI (11.99 %),and persistent DI (3.41%) have been reported in indicated percentages. While remission and resection rates have been reported as 60-80% in the literature. conclusion: In our series, rate of epistaxis was found to be higher than those reported in literature. Transient diabetes insipidus (DI), remission and resection rates were in accordance with the literature findings, while incidence of persistent diabetes was relatively lower. There was no majo...
