Modern clinical management of the patients sustaining traumatic injuries and thermal burns has resulted in their longer survival, but the clinical and pathological effects of these traumatic injuries over the myocardium have been largely neglected. It is speculated that certain factors such as the inflammatory and degenerative lesions of the heart, prolonged clinical course, and the subsequent stress and strain may play role in hastening the death. In the present study, 125 hospitalized cases of traumatic injuries and thermal burns brought for medicolegal autopsy were examined, with the purpose to find out the incidence, its significance, and the extent of the myocardial lesions due to stress and strain following trauma. About 20% patients had myocardial lesions recognized at gross and histological examination at autopsy. A myocardial lesion does develop in the cases of traumatic injuries and thermal burns. No significant sex difference is seen in the cases showing positive myocardial lesions. However, a relationship exists between these myocardial lesions and the after-effects developing in the cases of trauma. These myocardial lesions seen in the cases of traumatic injuries can be termed as early ischemic or anoxic lesions in the absence of any specific coronary pathology. The intensity of myocardial lesions increases with increase in the survival period of the patient. The findings in the study support the concept of human stress cardiomyopathy and demonstrate the potential significance of stress in precipitating death.
It is learnt from literature that neck injuries are encountered with different manner (suicide, homicide, accident). Sometimes homicidal neck injuries are pleaded or disguised as suicide e.g. murdered victims placed on railway track and even accidental neck injuries are disguised as homicidal to bring false charge of offence on enemies. Meticulous examination of neck injuries can give clues to settle these challenges (Suicidal, homicidal or accidental). Over all, suicide is the major cause of neck injury because of social, economic, work pressure, love failure; failure in exams, unemployment, marital disharmony and depression etc. Accidental deaths are an unplanned event occurring suddenly unexpectedly as unforeseen circumstance. The exponentially increasing number of automobile vehicles, poor adherence to traffic rules and regulations, abuse of alcohol and drug are the cause of accidents. Accidents tragically are not often due to ignorance, but are also due to carelessness, thoughtlessness and due to over confidence. Incidence of road traffic accidents has been increasing at the alarming rate throughout the world. Hence based on these reported data the present study was planned for the clinical evaluation of the pattern of different types of neck injuries in cases undergone autopsy. The present study was planned in Department of Forensic Medicine, Dr. Panjabrao Alias Bhausaheb Deshmukh Memorial Medical College, Shivaji Nagar, Amravati. Total 50 cases of the neck injuries undergone autopsy were evaluated in the present study. The data generated from the present study concludes that Neck injuries constitute a potential factor in increasing the amount of morbidity and mortality; therefore proper attention should be paid towards their accurate diagnosis and satisfactory management. Measurement of body size such as height and weight are required for assessment of growth and nutritional status of person, determination of basic energy requirements, standardization of measures for physical capacity, for adjusting drug dosage and for identifying an unknown cadaver. However, in some situations it is not possible to measure the stature of a person because of deformities of the limbs, in person who have undergone amputations or in unknown cadavers where lower limb (s) and / or trunk is mutated / absent. In such cases, stature has to be estimated using other body parameters. These estimations are also of prime importance in predicting the age-related loss in stature, identifying individuals with disproportionate growth abnormalities, skeletal dysplasia, medico-legal cases or height loss during surgical procedures on the spine. These measurements also have found application in normalizing pulmonary function in scoliosis. Hence based on these findings the present study was planned for evaluation of the stature height in the Maharashtra population by using arm span using regression equation. The present study was planned in Department of Forensic Medicine, Dr. Panjabrao Alias Bhausaheb Deshmukh Memorial Medical College, Shivaji Nagar, Amravati. The total 50 cases of the age group from 20 - 45 years were enrolled in the present study. This age group was chosen because the growth of an individual ceases by this age and there is no age related loss in body height at this age. Arm span measurement can serve as one of the most reliable body parameter to determine stature of an individual. It is useful in determining related loss in stature and in identifying individuals with disproportionate growth abnormalities. The data generated from the present study concludes that arm-span and hand length can be used in estimation of the height of both males and females. The regression equations so derived can be used in cadavers or an amputee with fairly accurate results. This can be helpful in medico- legal cases as well as in study of anthropology.
