Introduction: Glaucoma is a potentially blinding disease,causing gradual loss of sight. It is often associated with increased intraocular pressure that can damage the optic nerve, which transmits images to the brain. Glaucoma can lead to permanent vision loss, if the damage continues. Without treatment, glaucoma can cause total permanent blindness within a few years. Glaucoma is one of the major causes of ocular morbidity and primary open–angle glaucoma is a major health problem. The prevalence of primary open- angle glaucoma (POAG) is estimated as being from 1.1-3% of western populations, over the age of 40 years, in both past and more recent population surveys. Objective: To evaluate and compare the value of Contrast sensitivity (CS) and automated perimetry among primary open-angle glaucoma (POAG) patients, glaucoma suspects and normal control. Materials and Methods: It was a prospective observational hospital based study conducted in National Institute of Ophthalmology & Hospital (NIO&H), Dhaka during the period from January 2006 to December 2006. A total of 30 POAG patients, 30 glaucoma suspects and 30 normal control subjects were recruited for the study. After complete baseline evaluation, all the study subjects underwent visual field assessment by Octopus automated field analyzer and CS evaluation by Low Contrast Flip Chart. Outcome measures were baseline CS of POAG patients, glaucoma suspects and normal controls, mean CS of POAG patients at 3 months after treatment, baseline MS of POAG patients, glaucoma suspects and normal controls in Octopus perimetry and retinal mean sensitivity (MS) of POAG patients 3 months after treatment. Results: Among the POAG patients MS was 22.23 dB, among glaucoma suspects MS was 27.50 dB and among normal controls mean MS was 30.0 dB; among the POAG patients mean CS was 2.99% whereas it was 2.20% and 1.41% respectively among glaucoma suspects and normal controls. Baseline CS had negative correlation with MS among POAG patients (correlation co-efficient, r = -0.908). It indicates that better CS is associated with higher MS. Out of 30 cases of POAG, 20 (66.67%) received medical and 10 (33.33%) received surgical treatment. Mean CS improves to 2.66% from baseline 2.99% after treatment and MS improves to 23.16 dB from base 22.23 dB after treatment. Correlation after three months of treatment was significantly negative (r = -0.86). Conclusion: As increased loss of MS of perimetry has been found to be associated with increased loss of CS, assessment of CS may become an easy, convenient tool for glaucoma diagnosis. Introduction: Torsional ultrasound energy and burst mode interrupted energy delivery system are recent advancementsin the technique of phacoemulsification surgery. It has been reported that both these advancementshelpto reduce corneal wound burn and thermal induced endothelial cell loss and thereby helps to achieve excellent early postoperative visual outcome Objectives: To evaluate the efficacy of burst mode torsional phacoemulsification surgery in age-related cataract. Materials and Methods: Prospective observational study of 120 cases that underwent phacoemulsificationcataract surgery in which torsional energy was used in burst mode. Phacoemulsification was performed by “vertical –chop” technique. Intraoperatively corneal wound burn, Descemet’s membrane detachment, posterior capsule ruptureetc.were evaluated. Postoperatively corneal oedema, best corrected visual acuity, cystoid macular oedema were evaluated. Postoperatively eyes were examined at day 1, 1 week, 6 weeks and 3 months. Results: Mean age of the patients was 57.28± 8.20 years. Most of the patients 81(67.5%) had nuclear sclerosis grade-II cataract.Intraoperatively, moderate wound burn occurred in 02(1.6%) eyes, localized Descemet’s membrane detachmentin 03(2.5%) eyes and posterior capsule rupture occurred in 03(2.5%) cases. Postoperatively, moderate and severe corneal edema was found in 06(5.0%) and 04(3.3%) eyes respectively at day 1. At the end of 3 months follow-up, 114(95%) eyes maintained a best corrected visual acuity of ≥ 6/18, of which 109(90.8%) eyes achieved ≥ 6/9. Conclusion: Burst mode torsional phacoemulsfication is a very effective and advanced technique of cataract surgery. Intraoperative complications are very less and visual outcome is excellent. Purpose: Keratoconus is an ectatic corneal disease characterized by regional thinning and protrusion that leads to irregular astigmatism and, ultimately, substantial vision impairment. The aim of this study was to see if there was a link between serum IgE levels and keratoconus. Methods: At the Combined Military Hospital Dhaka, a prospective observational study was conducted in the general Eye OPD and Cornea clinic. The study involved 60 participants who were divided into 2 groups. Thirty people with keratoconus were assigned to Group A, while 30 healthy people were assigned to Group B. All 60 patients had their serum IgE levels checked. Results: The mean age of the study subjects in Group A and group B were 19.60±5.28 (SD), and 20.23±5.62 (SD) respectively; 13 were male and 17 were female in Group-A, and 10 were male and 20 were female in Group-B. Serum IgE was 243.183±143.059 (SD) in group-A and 86.067±77.065 (SD) in group-B. There was a significant rise in serum IgE in Group-A (p value <0.0001). Conclusion: Elevated serum IgE levels are strongly associated with keratoconus. Measuring serum IgE could be an important parameter in the management of Keratoconus. J. Natl. Inst.
