IntroductionPresence of vermiform appendix (non-inflamed or inflamed) in inguinal hernia is called Amyand's hernia in honor to surgeon C. Amyand who published the first case of perforated appendicitis within inguinal hernia in a boy caused by ingested pin. This presentation of foreign body Amyand's hernia appendicitis is very rare, and here we present such a case.Case presentationA 6-year-old boy, white Kosovar ethnicity, presented with right groin pain, swelling and redness. Two days before admission the patient was injured by football during a children game in the right lower abdomen and the next day he complained of pain in the right inguinal area.On admission patient had a painful non-reducible mass in the right inguinal region and cellulitis. Plain abdominal x-ray showed no fluid-air levels, but a metallic foreign body (pin) under right superior pubic ramus was apparent. With preoperative diagnosis of suspect incarcerated inguinal hernia with cellulitis the patient was operated on under general anaesthesia in December 2, 2006. Intraoperatively we found the inflamed vermiform appendix perforated by a pin in the hernial sac. Appendectomy and herniotomy were performed. The wound was primary closed, without any post-operative complications and follow up for the patient is three years long.ConclusionForeign body (pin) Amyand's hernia appendicitis seems to be extremely rare, maybe once in a century (Amyand 1735, Hall 1886, and our case in 2006). In patients with clinical signs of incarcerated inguinal hernia, with locally inflammatory signs, but without signs of intestinal obstruction Amyand's hernia appendicitis in differential diagnosis must be considered. In our case, it is possible that the injury during the football game might have induced perforation of the vermiform appendix with the foreign body in it.
Background:The obstructive hydronephrosis is a term that implicates the structural and functional changes of the kidneys as a result of difficulties in the flow of urine. Hydronephrosis specifically describes dilation and swelling of the kidney. Hydronephrosis is a condition that typically occurs when the kidney swells due to the failure of normal drainage of urine from the kidney to the bladder. Our aim was to evaluate the degree of hydronephrosis, causes and diagnostic method. Material andMethods:This is a study of 136 patients that have been treated at the Department of Urology, University Clinical Centre of Kosovo, Prishtina. For diagnosis of hydronephrosis in our patient, we used as equipment the Color Doppler ultrasound, with resolution of 3.5 MHz–8 MHz.Results:Out of 136 participants in the study, 91 (66.9%) were males and 45 (33.1%) females, with significant difference (P=0.000). The average age for males was 49 years old, whereas for females was 33. This study included patients with a diagnosis of symptomatic hydronephrosis with various causes and degrees. All patients were presented with hydronephrosis. The hydronephrosis grade varied from the stage I up to the IV. In our study we have difference grade of hydronephrosis, X2 test, P= 0.114. The most common causes of hydronephrosis in our study were; kidney stone, ureteral stones, neck stenosis PU, pregnancy, infiltrating bladder cancer, bladder neck contracture, prostate adenoma, infiltrating prostatic carcinoma etc. In this study we have indentified different causes, of which stones dominate as the most usual causes of hydronephrosis P= 0.0001.Conclusion:The Ultrasound is an easy method to be applied, non invasive, and a fast one to help and diagnose the obstructive hydronephrosis. The ultrasound has a high sensitivity and should be used as a screening method followed by other methods, as necessary. Hydronphrosis is most commonly presented to men with an average age about fifties. We came to the conclusion that the main causes of hydronephrosis are kidney stone, followed by ureteral stones, in which, in a larger percentage, they appear with the II degree of hydronephrosis.
We report the case of a 4-year-old girl presenting with the retroperitoneal epidermoid cyst. The lesion presented as an intra-abdominal cyst on physical examination and was followed up with more specific investigations by ultrasound and computed tomographic scanning. The final diagnosis was obtained only after laparotomy where the cystic mass was completely excised and pathological examination was done. The patient is well at 3-year follow-up. epidermoid cyst of the reteroperitoneal space, although rare, should be considered in the differential diagnosis of incidentally discovered intra-abdominal cysts during investigation of irrelevant illnesses or during routine abdominal ultrasound scan.
Background: Major trauma is a leading cause of death worldwide. Evaluation of trauma care using Trauma Injury and Injury Severity Score (TRISS) method is focused in trauma outcome (deaths and survivors). For testing TRISS method TRISS misclassification rate is used. Calculating w-statistic, as a difference between observed and TRISS expected survivors, we compare our trauma care results with the TRISS standard. Aim:The aim of this study is to analyze interaction between misclassification rate and w-statistic and to adjust these parameters to be closer to the truth. Materials and methods:Analysis of components of TRISS misclassification rate and w-statistic and actual trauma outcome.
ObjectiveThe objective of this study is to explore the feasibility and efficacy of a new technique for sutureless vascular anastomosis, using glued prosthesis, as a sole anastomosis fixation method in rabbits.MethodsTen rabbits were randomly selected to conduct the experiment. Five rabbits underwent direct anastomosis of infrarenal abdominal aorta, with glued prosthesis. In five other rabbits, reconstruction was done by sutured anastomosis. All animals were immediately examined by echo-Doppler for patency of anastomosis. The burst pressure of the glued anastomosis was measured and compared with that of a sutured artery. The animals were euthanized, and tissue samples were taken for histological examination immediately after the experiment.ResultsCompared to conventional anastomoses, sutureless vascular anastomoses required shorter time of creation and significantly reduced blood loss (P<5%). There was no significant difference on the average blood flow through the anastomosis between two groups at the end of surgery. All anastomoses with glued prosthesis, examined by echo-Doppler, were patent at the anastomotic site, except one, which was stenosed immediately after surgery. In the control group, except one with stenosis, all conventional anastomoses were patent. Mean burst pressure at the anastomotic site for sutureless anastomoses was lower than in control group. Macroscopically, the BioGlue did not demonstrate any adhesion to the surrounding tissue as it was covered by the vascular prosthesis. Histological examination showed low-grade inflammatory reaction in glued anastomoses versus no inflammatory reaction at the sutured anastomoses.ConclusionThis technique may provide a feasible and successful alternative in vascular surgery. However, further long-term studies are necessary to elucidate the break pressure and degree of inflammation at the anastomotic site.
