The endodontic triad consists of cleaning and shaping, disinfection and obturation. Success of root canal therapy in primary teeth is mainly achieved by thorough removal of debris and necrotic tissue. However, complete elimination of bacterial contaminants as well as necrotic debris require adjunctive use of root canal irrigants along with mechanical instrumentation. As we know from literature that none of the available irrigating solutions alone provides all the ideal requirements. Studies have shown that a combination of two or more irrigating solutions in a specific sequence will help to achieve optimal irrigation. This review article mainly highlights the mechanism of action, safety and biocompatibility of currently used irrigation solutions and also the other materials that can be used as a potent irrigants, their advantages and limitations in future of endodontics.
Cerebral palsy is defined as a nonprogressive brain disorder of movement and posture. The patient is characterized by mental and motor disabilities, sensory impairments of vision and hearing, besides having seizures and contraction joints. The dentist, doctors and healthcare professionals who treat patients with disabilities play an important role in health promotion and improvement in quality of life through multidisciplinary clinical planning especially in cerebral palsy patients who need special care. The aim of this case report was to describe a clinical case in which preventive and surgical interventions were performed in a 6-year-old male child with cerebral palsy under general anesthesia who reported to the Department of Pediatric and Preventive Dentistry, DAPMRV Dental College, Bengaluru, with the chief complaint of pain and swelling in upper left and lower right back tooth region. The approach targeted to the oral health of the patient with cerebral palsy can be considered as an important strategy for promoting the overall health and quality of life. A 6-year old boy was seen at the Paediatric clinic in Bangalore, with the chief complaint of pain in the lower left primary second molar. His medical, surgical, and personal history was noncontributory. Extraoral examination revealed left submandibular lymphadenopathy. Oral examination revealed a large carious lesion in relation to the left mandibular second primary molar. No soft tissue abnormalities were found. On careful clinical examination, extra additional cuspal projections were seen on the distobuccal surfaces of the right and left mandibular primary first molars [Table/ Fig-1]. The projections were 2x2mm in size with a globular shape and placed on the distobuccal surface of the mandibular primary first molars [Table/ Fig-2].The opposing teeth conformed well in occlusion with these cuspal projections. Odontogenic anomalies can occur as a result of conjoining or twinning defects. These include fusion, gemination and concrescence. The process of odontogenesis cannot be seen, therefore fusion and germination seems to be equivalent. This case report is of a boy aged-5-year who visited private dental clinic with a complaint of decayed upper front teeth. On clinical examination, the patient was diagnosed with ECC (Early childhood caries) and presence of fusion between primary upper left central incisor ( 61) and a supernumerary tooth. Radiographic analysis showed presence of a two roots with two root canals and external root resorption. The case was discussed and treatment planning was done to extract the tooth as a result of resorption in the apical region. The purpose of this report is to highlight the importance of diagnosing dental anomalies in the primary dentition, so as to organize a conservative individualized treatment plan to prevent complications during the child's formative years.
