The read data is Restructured by analytics software for evaluation and personalization in an aggregated form represented by an individualized health scoring and demographic grading system. The Results are previously restructured data that must become actionable to lead to an escalation path to the Communication Center for a Response. On a periodic basis, Repeat of the cycle will improve patient health outcomes through an interconnected link of services for optimal continuous health relationship management. Providers can leverage the HRMS platform to generate data-driven approaches fostering a positive and lasting impact on their patient's health. Over the past several decades, healthcare services have drastically changed, altering the manner in which healthcare was previously managed. Technological advancements in medical systems have revolutionized the healthcare industry, and digital health tracking has been quite successful in monitoring patients' health. Since patients are continuously monitored, no matter where they are, these systems can indicate patients' adherence to medical protocols and act as a warning sign for such diseases as heart problems, Alzheimer's disease, and many others. Health Relationship Management Services (HRMS) is a new paradigm which defines comprehensive healthcare for an individual. HRMS is a complete health ecosystem suitable for the workplace, which enables healthcare providers to collect personal health data from various sources, analyze it for positive outcomes, and take action to preserve an employee's good health to reduce absenteeism or turnover. There is so much health and medical information available today that physicians cannot be expected to know it all. Thus, advances in technology have become a necessity for doctors to track patient information and care, and add to patient databases for reference and to conduct research. It is important to understand the new language of digital health, such as Personal Health Record (PHR), Electronic Medical Record (EMR) and Electronic Health Record (EHR), all of which sound similar, but are not interchangeable. The ideal comprehensive IT system would empower patients, advance healthcare delivery and transform patient data into life-saving research (Kaiser, 2015). OmniFluent Health is language translation software that will allow for better patient/ practitioner communication and avoid errors. Digital technology employs the use of big data that is shared, accessed, compiled and applied using analytics. However, information transfer, especially as mandated by current ethics of use of technology, has resulted into breach of patient privacy. Improved digital technology is providing the health care field with upgrades that are necessary, electronic files and health records, from mobile apps, and remote monitoring devices.
