Content inflamed within right inguinal hernia is also known as Amyand’s hernia, is a rare clinical condition characterized by the similar complaints of an appendicitis. In case of strangulation or acute appendicitis occurring inside the hernia sac, the condition may be asymptomatic or manifest as an incarcerated hernia. Every surgeon should be ready to handle such an unforeseen event because the majority of these unusual instances are treated by urgent surgery without a preoperative diagnosis which makes this report informative. Background: Recurrent laryngeal nerve injury, hypothyroidism and hypocalcaemia have long been recognized as the three main sequalae of thyroidectomy. Persistent hypocalcaemia may cause intracranial lesions and cardiac arrhythmias. As the definitive diagnosis of Hashimoto’s thyroiditis was not always possible with clinical and cytological parameters when antibody testing was not done, surgery was planned with the diagnosis of multinodular goitre. This study was conducted to find out the incidence of post-operative hypocalcaemia following total thyroidectomy by skilled surgeons in patients with multinodular goitre and Hashimoto’s thyroiditis.Methods: This was a cross-sectional study to compare the incidence of hypocalcaemia in patients with biopsy-proven Hashimoto’s thyroiditis and multinodular goitre. After obtaining approval from the institutional review board and ethics committee, data was collected serially, from the surgical records of 123 patients who underwent total thyroidectomy in this tertiary care centre, during the past two years.Results: The incidence of post-operative hypocalcaemia (71.1%) in patients undergoing total thyroidectomy with Hashimoto’s thyroiditis was significantly higher than in patients with multinodular goitre (56.4%). Of the 45 patients who had a biopsy report of Hashimoto’s thyroiditis, 8 (17.8 %) developed permanent hypocalcaemia while only 8 (10.3%) of the 78 patients with histopathological diagnosis of multinodular goitre developed persistent hypocalcaemia needing supplementation following total thyroidectomy in this rural tertiary care hospital.Conclusions: The significant risk of post-operative hypocalcaemia should be kept in mind before opting for total thyroidectomy for benign lesions of the thyroid and pre-operative antibody studies must be done to diagnose Hashimoto’s thyroiditis.
INTRODUCTION: Obesity is one of the most prevalent disorders of the world. Obesity possesses an increased risk for both elevated IOP and systemic vascular abnormalities such as hypertension and arteriosclerosis. There are conflicting reports regarding the relationship between BMI and IOP. There is a need to evaluate these contradicting findings in our environment. AIM: To determine the relationship between intraocular pressure and body mass index, in diabetics and nondiabetic adults in Indian population. MATERIALS AND METHODS: The present study was a crosssectional analysis based on data obtained at a one day screening programme of a general population in an Eye camp conducted in Mysore. 115 subjects were studied after obtaining an informed consent. Participants underwent standardized examinations including anthropometric indices, measurement of blood pressure, rebound tonometry and ocular examination including fundoscopy and intra ocular pressure. Random blood sugar was measured for all the subjects. RESULTS: out of 115 subjects, there were 90 males (78.26%) and 25 females (21.73%). The mean age was 44.34 years +/-11.75 years. 31 (26.95%) of them were diabetic and 84(73.04%) of them were non-diabetic. Maximum number of males i.e 42(46.66%) were overweight and 9 females (36%) were overweight. Out of 51 overweight subjects, 41(80.4%) had IOP < 21mmHg and 10(19.6%) had IOP >21 mmHg. Out of the 23 obese subjects, 4(17.4%) had IOP < 21mmHg and 19(82.6%) had IOP>21mmHg which was statistically significant. The mean IOP amongst diabetics was 18.45mmHg +/-3.8 mmHg and amongst nondiabetics was 19.16 mmHg +/-3.92 mmHg which was not statistically significant. CONCLUSION: In a population of 115 screened, there was a significant correlation between IOP and BMI but there was no statistically significant relationship between BMI and diabetes.
