Background Intramuscular adipose tissue (IMAT) of the lower extremity is a strong negative predictor of mobility function. Variability in temporal gait factors is another important predictor of mobility function. The purpose of this study was to examine the relationships between IMAT in the hip and thigh muscles, balance, and temporal gait variability in older adults. Methods Forty-eight healthy community dwelling older adults (74 +/− 1 year) underwent a CT scan to quantify IMAT in the gluteus maximus (Gmax), gluteus medius/minimus (Gmed/min), hamstrings, vastus lateralis, and adductor muscles. Temporal Gait measures were collected on a GAITRite walkway and gait variability was determined by calculating intra-individual standard deviations. Individuals were divided by tertiles of temporal gait variability into categories of high, medium, and low variability. Differences in the IMAT of the hip abductors were calculated for those with high and low gait variability and partial correlations for gait variability and all muscle composition measures were determined for all variables with normalized gait speed as a covariate. Results Gmed/min IMAT was greater in those with higher gait variability compared to those with lower gait variability (p<0.05). Gmed/min IMAT was related to stride width variability (r=0.30, p<0.05). Gmax IMAT was also related to time variability of swing (r=0.42), stance (r=0.26), double limb support (r=0.43), double support loading (r=0.44), and double support unloading (r=0.50) (all p<0.05). Conclusion Increased IMAT in the proximal hip muscles, particularly the hip abductors, was associated with increased gait variability and poorer balance. These findings may have implications for the assessment and treatment of balance and falls such that interventions for enhancing balance and mobility among older individuals should take into account the importance of gluteal muscle composition.
To demonstrate practical longitudinal functional outcome measurement for rehabilitation treatment planning, patient education, and program management. Study Design: Descriptive analysis of consecutive discharges from an accredited inpatient rehabilitation program across 3 time epochs (inpatient discharge and 6 and 12 months postdischarge). Setting: Midwestern Medical Center. Participants: 541 persons with cerebrovascular accident (n ϭ 379), acquired brain injury (n ϭ 89), or spinal cord dysfunction (n ϭ 73). Main Outcome Measures: SF-36 Health Survey. Results: Graphical profile plots demonstrated differential patterns and trajectories of recovery for group and individual patients. Conclusions: The SF-36 provides valuable outcomes information for medical rehabilitation, with practical utility for serial longitudinal assessment. Such use can foster understanding of differential outcomes for diagnostic groups and the individual rehabilitation consumer.
Introduction Despite knowledge about major health effects of secondhand tobacco smoke (SHS) exposure, systematic incorporation of SHS screening and counseling in clinical settings has not occurred. Methods A three-round modified Delphi Panel of tobacco control experts was convened to build consensus on the screening questions that should be asked and identify opportunities and barriers to SHS exposure screening and counseling. The panel considered four questions: (1) what questions should be asked about SHS exposure; (2) what are the top priorities to advance the goal of ensuring that these questions are asked; (3) what are the barriers to achieving these goals; and (4) how might these barriers be overcome. Each panel member submitted answers to the questions. Responses were summarized and successive rounds were reviewed by panel members for consolidation and prioritization. Results Panelists agreed that both adults and children should be screened during clinical encounters by asking if they are exposed or have ever been exposed to smoke from any tobacco products in their usual environment. The panel found that consistent clinician training, quality measurement or other accountability, and policy and electronic health records interventions were needed to successfully implement consistent screening. Conclusions The panel successfully generated screening questions and identified priorities to improve SHS exposure screening. Policy interventions and stakeholder engagement are needed to overcome barriers to implementing effective SHS screening. Implications In a modified Delphi panel, tobacco control and clinical prevention experts agreed that all adults and children should be screened during clinical encounters by asking if they are exposed or have ever been exposed to smoke from tobacco products. Consistent training, accountability, and policy and electronic health records interventions are needed to implement consistent screening. Increasing SHS screening will have a significant impact on public health and costs.
