OBJECTIVE The authors evaluate the rate and discuss the pathomechanisms of very late (≥ 4-month) ischemic complications after flow-diverter stent (FDS) placement for intracranial aneurysms. METHODS The authors retrospectively reviewed the clinical data of the patients treated at Pitié-Salpêtrière Hospital between January 2010 and September 2014, who underwent FDS placement for intracranial aneurysm. The patients received dual-antiplatelet therapy (clopidogrel and aspirin) 5 days before and 3-6 months after the procedure and then aspirin alone for 6-9 months. An ischemic complication was defined as a sudden focal neurological deficit documented on diffusion-weighted images. RESULTS Eighty-six consecutive patients were included. Three (3.5%) patients treated with the Pipeline embolization device experienced a delayed acute ischemic stroke (2 cases of perforator/side-wall branch infarction and 1 case of thromboembolic stroke) with an average delay of 384 days (4 months, 20 months, and 13 months, respectively). The aneurysm locations were the left superior cerebellar artery, the right anterior choroid artery, and the left internal carotid artery (paraclinoid segment), respectively. The complications occurred after the patients had completed the antiaggregation protocol, except for Patient 1, who was receiving aspirin alone because of a spontaneous hematoma. At the acute phase, no in-stent thromboses were found on digital subtraction angiography. In Patient 2, the treated anterior choroid artery was occluded 20 months after the procedure. In Patient 3, a focal stenosis (approximately 40%) of the distal aspect of the FDS, probably caused by intimal hyperplasia, was seen. CONCLUSIONS Very late ischemic complications after FDS treatment were observed in 3.5% of the cases in the authors' series, some of which occurred as late as more than 1 year after placement.
A 22-year-old man presented with a sudden backache and paraplegia (ASIA = B). Magnetic resonance imaging showed an anterior pan-spinal epidural haematoma. Digital subtraction angiography was performed and ruled out an underlying vascular malformation but showed an active contrast media leakage into the T-4 ventral epidural space with a pattern of pseudo-aneurysm. A rupture of a T-4 retrocorporeal artery was considered as the aetiology, possibly caused by a haemorrhagic sub-adventitial dissection. Treatment consisted in the embolisation of both the pseudo-aneurysm and the parent artery with liquid acrylic glue, followed by neurosurgical decompression in emergency. The patient had totally recovered (ASIA = E) by the 10-month clinical follow-up.
Introducción: La fístula vesico-vaginal es la fístula adquirida más común de las vías urinarias; su principal manifestación es la salida de orina por vía transvaginal. Una fístula representa una comunicación anormal entre dos superficies epitelizadas. La clasificación de fistulas se basa en el órgano de origen en el tracto urinario y el punto de terminación de la fístula. La incontinencia urinaria debida a la formación de una fistula supone un importante problema higiénico, psicológico y social para la paciente, debido a la continua sensación de humedad, olores indeseables, infecciones vaginales, vesicales y a la incapacidad laboral y social que le condiciona. Presentación de caso: Paciente de 44 años de edad, antecedente de histerectomía, por dismenorrea y miomas uterinos, con pérdida de orina transvaginal 4 semanas posteriores a la cirugía, se realiza reparación de fistula 4 meses posteriores a su cirugía, se realiza por abordaje laparoscópico transperitoneal, por 4 puertos, con un seguimiento a 6 meses, con resolución completa, sin recurrencia, con sangrado mínimo, actualmente sin catéteres, con función genital y urinaria normal, con adecuada continencia urinaria. Conclusión: El abordaje laparoscópico es una excelente alternativa, aunque se requiere experiencia en cirugía laparoscópica pélvica, con el beneficio para el paciente, de recuperación rápida, con menor dolor y sangrado, por esto creemos que con practica y paciencia es perfectamente realizable este tipo de abordaje.
Son neoplasias suprarrenales benignas compuestas por tejido adiposo y hematopoyético. Corres`ponden a 6 - 16% de incidentalomas suprarrenales, su tamaño va desde milímetros hasta > 10 cm, y luego se denominan adrenomielolipomas gigantes. Síntomas de presentación comunes son dolor: abdominal: 22,5%, en hipocondrio: 13,9%, en flanco: 13,9%, masa abdominal: 5,2%, y raramente: disnea, dolor en espalda, fiebre, pérdida de peso y virilización. Masculino 61 años de edad, con tumor 15 x 12 cm, en glándula suprarrenal derecha, comprime polo superior de riñón derecho, de -20 UH, compatible con mielolipoma suprarrenal derecho gigante, hallazgo incidental durante estudio por crecimiento prostático, se realiza resección por abordaje laparoscópico, con excelentes resultados clínicos, sangrado mínimo y recuperación inmediata Es benigno y se diagnostica incidentalmente, no es funcional, pero se deben hacer estudios hormonales preoperatorios. El tratamiento depende de cada caso, se recomienda cirugia de tumores grandes >5 cm, sugerimos abordaje laparoscópico.
