According to the World Health Organization (WHO, 2018), patient safety is a major public health problem worldwide. There is a 1 in 300 chance that a patient will be injured while receiving health care (WHO, 2018). Using conservative estimates, recent data show that patient harm is the fourteenth main cause of morbidity and mortality worldwide (Jha et al., 2013). Although patient harm during hospital admissions is the most widely studied situation, recent literature reviews have observed that medical errors in primary care occur between 5 and 80 times per 100,000 consultations (Sandars & Esmail, 2003). Safety lapses in primary and ambulatory care are, therefore, common and in many cases avoidable. It has been estimated that in developed and developing countries, up to 20% or 25%, respectively, of the general population experience harm in the health care setting
