Introduction: Third molar surgery can cause post-operative complications to the patient due to the presence of wound from the incision. The study aimed to compare modified triangular (triangular reverse) flap with triangular and envelope flap on postoperative complications by the measurements on post-surgical swelling, trismus, pain scale, wound dehiscence and the occurrence of alveolar osteitis. Methods: Study design using a single-blind randomized clinical trial. Each treatment group consisted of 10 patients. An assessment was performed on day 1, 7 and 14 after surgery. The swelling was measured using different anatomical points, and trismus was measured using the distance of inter-incisal opening. The occurrence of dehiscence was measured from the width of the incision line. Alveolar osteitis and pain scale was observed. Results: The results of one way ANOVA (p > 0.05) shows there was no difference between treatment groups both for swelling and mouth opening, but there was a difference between treatment groups for dehiscence on days 7 and 14. The results of the Post Hoc test show that flap envelope differs from the other flap design, whereas triangular and triangular reverse flap showed no difference. Friedman test results (p > 0.05) showed no difference in VAS score. There was no alveolar osteitis occurred in each treatment group during postoperative control. Kruskal Wallis test (p > 0.05) showed that there was no difference of VAS score between treatment group during post-operative control. Conclusion: The triangular reverse flap design may decrease the occurrence of postoperative complication after mandibular third molar surgery.
The quality of life of patients becomes an essential part of the success of an intervention since it is used as an outcome measure. Mandible resection will cause discontinuities that can degrade the quality of life of patients. Extensive resectionand reconstruction will affect the quality of life of patients after resection and reconstruction surgery as compared with that before the surgery. On this basis, cross-sectional study was conducted among 27 patients suffering from benignmandibular tumors undergoing surgical procedure of marginal resection, segmental resection, or hemimandibulectomy at Dr. Sardjito genral hospital in 2010-2015. Quality of life of patients after mandibular resection and reconstruction was measured with modified oral health impact profile (OHIP-14). An assessment of the quality of life before and after surgery was based on gender and type of reconstruction and it was analyzed using t-test and one-way ANOVA. It was revealed that the quality of life of patients with benign tumors increased significantly (p=0,000), and that gender differences did not significantly affect the improved quality of life (p=0.433). The mean score of patient’s quality of life was highest on the type of marginal resection (mean value =14.50), and the lowest was on segmental resection (mean value=7.50), but the type of resection did not significantly influence the improved quality of life (p=0.152). Resection and reconstruction procedures under taken by Oral and Maxillofacial Surgeon at Dr. Sardjito general hospital can improve the quality of life of patients with benign tumors of the lower jaw.
Background: Silkworm cocoons are materials that have fine regenerating abilities for the human body. Fibroin and sericin present in silkworm cocoons (Bombyx mori) are thought to have anti-inflammatory effects. This study aimed to know the effects of the use of wound dressing from silkworm cocoons toward COX-2 expression and neutrophils number in the inflammatory phase after skin excision. Methods: Twelve male Wistar rats according to inclusion criteria were randomly divided into 4 groups, each group of 6, based on the decapitation time (the 3rd day and the 6th day) and based on the dressing material (moist gauze dressing as the control group and silkworm cocoons as the treatment group). Each group was performed an excision on the dorsal skin with subcutaneous depth using a 4 mm-round punch biopsy. Neutrophil cell observations were performed by Hematoxylin eosin staining (HE). COX-2 expression was found in preparations for immunohistochemical staining using rabbit monoclonal COX-2 antibody at sacrificed period on the 3rd and the 6th day after wound dressing application. Results: The number of neutrophils and expression of COX-2 were analyzed using Two-way ANOVA and Independent t-test. The results showed a significant decrease in the number of COX-2 expression on inflammatory cells as well as the number of neutrophils (p<0.005) in the groups treated with wound dressing from silkworm cocoons on both the 3rd and 6th day. Conclusions: It was concluded that the use of wound dressing from silkworm cocoons can inhibit COX-2 expression (p=0,000) and decrease the number of neutrophils in the inflammatory phase after skin excision (p=0,001).
ABSTRAKKasus bibir dan lelangit sumbing merupakan salah satu kelainan deformitas yang sering terjadi. Keadaan klinis bervariasi mulai dari kasus sumbing tidak komplit sampai dengan komplit. Keadaan klinis dengan lebar celah yang bervariasi membutuhkan teknik pembedahan yang tepat. Pasien-pasien dengan bibir dan atau lelangit sumbing mungkin sebelumnya telah menjalani beberapa intervensi pembedahan, sehingga sering memerlukan koreksi lebih lanjut untuk memperbaiki hasil operasi sebelumnya. Pasien anak laki-laki usia 4 tahun 7 bulan mengeluhkan masih terdapatnya celah di lelangit. Pasien didiagnosis dengan labiognatopalatoschisis bilateral. Sebelumnya pasein sudah menjalani 4 kali operasi penutupan celah bibir dan lelangit. Pasien menjalani 2 kali operasi bibir sumbing dengan metode lip adhesion dan metode Barsky, dan 2 kali operasi lelangit sumbing dengan metode pushback dilanjutkan dengan koreksi dengan z plasty, tetapi hasil akhir masih terdapat celah di palatum mole. Selanjutnya dilakukan operasi repair palatoplasi dengan metode Furlow double opposing z plasty dengan kombinasi insisi lateral, dan didapatkan hasil menutupnya celah di palatum mole sampai dengan uvula. Telah dilakukan operasi repair palatoplasi dengan metode Furlow double opposing z plasty. Teknik ini dilakukan untuk menghindari insisi yang terlalu luas dikarenakan terdapatnya jaringan fibrous yang tebal pada mukosa palatum pasca operasi sebelumnya. There are variety of clinical appearance ranging from incomplete to complete cases. Clinical appearance with different width requires proper surgical technique. Patients with cleft lip and palate had undergone surgical intervention, so that they needed surgical correction to repair the result or failure of the previous surgery. A Four year old boy complain there was cleft on the soft palate. The patient was diagnosed with labiognatopalatoscisis. The patient had undergone two stages of cleft lip surgery and twice of cleft palate surgery with pushback method and repair with z plasty, however the result was unsatisfactory. Further, the patient underwent repair palatoplasty surgery with Furlow double opposing z plasty method combined with lateral relaxing insicion. The result in the post surgery was the closure of cleft soft palate up to uvula. Repair palataplasty surgery has been done with Furlow double opposing z plasty method. This technique could avoid extended incision due to thick fibrous tissue on the palatum mucosa as the result of serial previous surgery.
