Background and Objectives:Alpha-2 agonists are mixed with local anaesthetic agents to extend the duration of spinal, extradural and peripheral nerve blocks. We compared clonidine and dexmedetomidine as an adjuvant to local anaesthetic agent in supraclavicular brachial plexus block with respect to onset and duration of sensory and motor block and duration of analgesia.Methods:Sixty ASA I and II patients scheduled for elective upper limb surgeries under supraclavicular brachial plexus block were divided into two equal groups in a randomized, double-blinded fashion. Group C received clonidine 1 μg/kg and Group D received dexmedetomidine 1 μg/kg added to bupivacaine 0.25% (35 cc). Onset and recovery time of sensory and motor block, duration of analgesia and quality of block were studied in both the groups.Results:Duration of sensory block and motor block was 227.00±48.36 and 292.67±59.13 min, respectively, in group C, while it was 413.97±87.13 and 472.24±90.06 min, respectively, in group D. There was no statistically significant difference in onset of sensory and motor block between the two groups. The duration of analgesia (time to requirement of rescue analgesia) in group D was 456±97 min, while in group C, it was 289±62 min. Statistically, this difference was significant (P=0.001). The number of patients achieving grade IV quality (excellent) of block was higher in group D (80%) as compared with group C (40%) (P<0.05).Conclusion:Dexmedetomidine when added to local anaesthetic in supraclavicular brachial plexus block enhanced the duration of sensory and motor block and also the duration of analgesia. The time for rescue analgesia was prolonged in patients receiving dexmedetomidine. It also enhanced the quality of block as compared with clonidine.
Purpose Offering unskilled people training in engineering and vocational skills helps to decrease unemployment rate. The purpose of this paper is to augment actual hands-on conventional vocational training methods with virtual haptic simulations as part of computer-based vocational education and training. Design/methodology/approach This paper discusses the design of a bi-manual virtual multi-modal training interface for learning basic skills in surface mount device hand soldering. This research aims to analyze human hand dexterity of novices and experts at micro level skill knowledge capture by simulating and tracking the users’ actions in the manual soldering process through a multi-modal user interface. Findings Haptic feedback can enhance the experience of a virtual training environment for the end user and can provide a supplementary modality for imparting tangible principles to increase effectiveness. This will improve the teaching and learning of engineering and vocational skills with touch-based haptics technology, targeted toward teachers and students of various disciplines in engineering. Compared with the traditional training methods for learning soldering skills, the proposed method shows more efficiency in faster skill acquisition and skill learning. Originality/value In this study, the authors proposed a novel bi-manual virtual training simulator model for teaching soldering skills for surface mount technology and inspection. This research aims to investigate the acquisition of soldering skills through virtual environment, with and without haptic feedback. This acts as a basic-level training simulator that provides introductory training in soldering skills and can help initially unskilled people find educational opportunities and job offers in the electronics industry.
The coronavirus pandemic has affected millions of lives around the world. The availability of personal protective equipment at a low cost has been an important factor in determining the well-being of frontline health workers who are integral to the global response to COVID-19. This paper discusses the design and development of a low-cost open-source powered air-purifying respirator (PAPR) for medical professionals which costs approximately INR 10,000. We detail the assembling process for this respirator using widely available components that allows for easy scale-up and replication in any country, thereby ensuring the safety of frontline health and sanitation workers who are at an elevated risk of contracting COVID-19. We worked with doctors at Amrita Institute of Medical Sciences to test the prototype for airflow under varying battery life conditions and found it to provide 10 CFM airflow (1.7 times the National Institute for Occupational Safety and Health (NIOSH) airflow requirements) for over 7 hours without any long term effects due to noise from the actuators.
Due to the current pandemic, governments all over the world have given stay at home orders and have advised people to follow precautions like social distancing. These measures have a significant impact on our social, economic, and political setting which ultimately affects the volume and distribution of crimes worldwide. To understand the impact of social distancing policies on crime, we have compared the crime statistics during the pandemic (2020) with that of the previous year (2019) for the two metropolitan cities New Delhi, India, and New York, USA. We observe the trends of different types of crime and conclude that while crimes like residential burglaries have reduced, crimes like domestic violence have increased significantly. A rise in commercial burglaries 1 was observed in New York whereas in New Delhi a decline was observed because of the presence of the police on roads. Furthermore, A decline in crimes like robbery, burglary, larceny, etc. was observed whereas a sharp increase was seen in the cases of domestic violence in both the cities.
