Laryngomal acia is the most frequent cause of stridor in childhood, and in most of the cases, spontaneous resolution occurs by the age of 2 years. Approximately 10% of the cases (severe laryngomalacia) require surgery. This condition is of unknown etiology and its diagnosis is made by fiberoptic laryngoscopy, which shows shortening of the aryepiglottic folds, and/or redundant arytenoid mucosa, and/or anterior-posterior epiglottic prolapse. Aim: Our objective was to verify the main clinical and anatomical affections and to highlight the clinical parameters for clinical follow-up and surgical indication in patients with laryngomalacia. Study design: Transversal cohort study. Material and Method: Twenty-two children diagnosed with laryngomalacia in the Pediatric Otorhinolaryngology of UNIFESP-EPM, from January 2001 to December 2003, whose clinical and surgical follow-up were performed by the same examiner, were enrolled in this study. Results: Out of twenty-two evaluated children, 2 (9.1%) presented with severe laryngomalacia and pectus excavatum (funnel chest). At polysomnography, no child presented any significant respiratory event during sleeping. Those two children with severe laryngomalacia were submitted to supraglottoplasty with resection of the aryepiglottic folds. Conclusion: We concluded that stridor and shortening of the aryepiglottic folds are preponderant in children with laryngomalacia. The polysomnographic exam did not prove to be a good parameter for clinical follow-up, neither for surgical indication. The most important parameters were pectus excavatum and failure to thrive. Supraglottoplasty is effective and has low morbidity rate.
Bel l's palsy is a unilateral facial paralysis of sudden onset and unknown cause. It may affect salivation, taste and lachrymation depending on the site of facial nerve involvement. Patients can report supersensitive hearing. The stapedius reflex is absent in patients with Bell's palsy. Aim: The objective of the present study was to check if patients with Bell's palsy present hyperacusis. Study design: Clinical prospective. Material and Method: Eighteen patients with peripheral facial paralysis were randomly selected and examined. Complete ENT evaluation was performed, including Hilger facial nerve stimulator, Schirmer's test, electrogustometry, pure tone testing, speech audiometry, immittance testing and discomfort loudness levels. The group aged 31-40 years was the most affected by peripheral facial paralysis in this sample. Results: The incidence was higher in females (61%). The right side of the face was involved in 56% of patients. As to local involvement, grade IV was observed in 44% of cases and grades III and V in 28% of patients each. Only one patient (5.5%) complained of hyperacusis. All studied patients presented reduced tolerance threshold in the audiometric graphs, and stapedius reflex protects these patients by 16dB on average. Conclusion: Therefore, we could conclude that the frequency of complaints of hyperacusis in patients with Bell's palsy was similar to that of the general population; however, in audiometric terms, the tolerance threshold in the paralyzed side was lower when compared with the normal side.

Evi dence of a relation between gastroesophaeal reflux and pediatric respiratory disorders increases every year. Many respiratory symptoms and clinical conditions such as stridor, chronic cough, and recurrent pneumonia and bronchitis appear to be related to gastroesophageal reflux. Some studies have also suggested that gastroesophageal reflux may be associated with recurrent laryngeal papillomatosis, contributing to its recurrence and severity. Aim: the aim of this study was to verify the frequency and intensity of gastroesophageal reflux in children with recurrent laryngeal papillomatosis. Material and Methods: ten children of both genders, aged between 3 and 12 years, presenting laryngeal papillomatosis, were included in this study. The children underwent 24-hour double-probe pH-metry. Results: fifty percent of the patients had evidence of gastroesophageal reflux at the distal sphincter; 90% presented reflux at the proximal sphincter. Conclusion: the frequency of proximal gastroesophageal reflux is significantly increased in patients with recurrent laryngeal papillomatosis.
