Background: Post-operative complications following appendectomy are relatively not uncommon and pain during this period is sometimes seen. Persistent or reappearance of similar pain causes the loss of patient’s confidence on the procedure itself as well as despair for surgeons.Methods: The study was conducted at our institution to determine the cause of post appendectomy right iliac fossa pain and to evaluate the role of laparosopic adhesiolysis as a therapeutic tool. 35 patients with post appendectomy right iliac fossa pain were included in the study and outcome of post-operative peri-caecal adhesions (if found) treatment with laparoscopic adhesiolysis was assessed.Results: 80% of the 35 patients were in the age group of 18-38 years. 37.14% (13 patients) were symptomatic within 1 year of appendectomy. Pain due to post-operative adhesion was found in 20% (7) of the patients. With laparoscopic adhesiolysis, the diagnostic accuracy rate was 78.57% and therapeutic relief in pain was 85.72% of the patients in our study.Conclusions: Patients presenting recurrent pain in right iliac fossa, after appendectomy should not be overlooked. Patients with recurrent right iliac fossa pain following appendectomy may benefit from laparoscopy, both as a diagnostic tool and with the added advantage of treating the patients simultaneously in the form of laparoscopic adhesiolysis. Adhesiolysis has offered pain free life as long as our follow up is concerned.
BACKGROUNDThe advances in groin hernia repair in the century following Bassini have shared the primary goal of reducing long-term hernia recurrence rates. The surgeon must have a comprehensive understanding of the anatomy of the groin to properly select and utilise various options for hernia repair.The aim of our study was to compare the Grid-Iron technique and the Lichtenstein method of open hernia repair in terms of operative time, post-operative pain, post-operative complications, hospital stay, return to normal activity and early recurrence rate. MATERIALS AND METHODSThis prospective randomised comparative study period was conducted from August 2015 to July 2017 with a sample size of 60 cases in Kalyani Medical College and included patients who reported to outpatient department. Patients were randomised into two groups, one group underwent repair of inguinal hernia by Grid-Iron technique and the other by Lichtenstein technique. RESULTSThose who underwent Grid-Iron technique of repair had less intensity post-operative pain, shorter duration of hospital stay and returned to work earlier; whereas those who had undergone Lichtenstein repair had less post-operative complications and had no recurrence unlike in other group which had two recurrences. CONCLUSIONLichtenstein repair of inguinal hernia is a time-tested method of treatment of inguinal hernia. It is associated with the lowest rate of recurrence among the various methods of repair. Being tension-free it allows the patient to early return to previous state of activity, whereas Grid-Iron technique is considered a more physiological repair having all the advantages of Minimal Access Surgery like of quicker return to work, less post-operative pain and a better cosmetic result.
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a highly infectious and pathogenic virus. To date, there is a lack of proper medication against this virus, which has triggered the scientific community to find therapeutics. Searching of SARS-CoV-2 main protease inhibitors from anti-viral natural products based on traditional knowledge may be an effective approach. In this work, structure-based virtual screening of the compounds of Justicia adhatoda was performed against SARS-CoV-2 Mpro, followed by ADME filtration, molecular dynamics, and MMGBSA-based binding free energy calculation. On the basis of docking score, crucial interacting amino acid residues, molecular dynamics, and binding energy profile, three novel phenolic compounds JA_38b, JA_38c, and JA_39 were selected as potential binders against SARS-CoV-2 Mpro. This information may be used to develop potential therapeutics countermeasures against SARS-CoV-2 infection after in vitro and detailed pharmacological study.
