Objectives Burnout and occupational stress are frequently experienced by healthcare professionals (HCPs). Mindfulness-based stress reduction (MBSR) has been found to improve the psychological health outcomes of HCPs. To date, systematic reviews and meta-analyses have primarily focused upon empirical investigations into the reduction of stress amongst HCPs using MBSR and are limited to empirical studies published before December 2019. This systematic review aimed to update the current evidence base and broaden our understanding of the effectiveness of MBSR on improving the psychological functioning of HCPs. Methods Three electronic databases (Medline, Psych Info and Web of Science) were searched without time frame restrictions. Quantitative studies included randomised controlled trials, clinical controlled trials, pre-post designs and studies with up to a 12-month follow-up period. All studies included in the review employed a MBSR programme, standardised measures of psychological functioning and qualified HCPs as participants. Results Using PRISMA guidelines thirty studies were included in the review. The reviewed literature suggested that MBSR was effective in reducing HCPs experiences of anxiety, depression and stress. MBSR was also found to be effective in increasing HCP levels of mindfulness and self-compassion. However, MBSR did not appear as effective in reducing burnout or improving resilience amongst HCPs. Abbreviated MBSR programmes were found to be as effective as the traditional 8-week MBSR programmes. Conclusions MBSR is an effective intervention which can help improve the psychological functioning of HCPs. Recommendations include improving the overall quality of the studies by employing more robust controlled designs with randomisation, increased sample sizes with heterogeneous samples, and making active comparisons between interventions used.
Whilst benefits of an external focus are shown to govern several characteristics of skill execution, specificity theory indicates that sources of afferent information most useful to performance execution are typically prioritised during processing.Objectives: We investigated whether an internal focus facilitates performance when pertinent afferent information is proprioceptive in nature and congruent with attentional focus. We also considered whether the mechanisms behind attentional focus differences are attributable to planning processes or online motor control.Design: Experiments 1 and 2 adopted a randomised design, whilst experiment 3 used a repeated measures approach.Method: In Experiment 1 we investigated movement variability as a measure of planning and error correction under external and internal focus conditions in an aiming task. Experiment 2 removed visual information to increase pertinence of proprioceptive feedback for movement execution and Experiment 3 adopted a leg-extension task, where proprioceptive salience was enhanced using an ankle weight. We hypothesized that this would increase congruency between internal focus instructions and movement production.Results: Experiments 1 and 2 revealed reduced amplitude errors under an internal focus whilst Experiment 3 showed similar findings with the addition of lower EMG activity when adopting an internal focus. Movement variability findings were indicative of enhanced planning.Conclusions: When pertinence of proprioceptive information was amplified, benefits of an internal focus were more pronounced and performance was higher. Participants were better able to focus on movement characteristics to process proprioceptive feedback: something not afforded under an external focus. This raises doubts regarding the rigidity of the constrained action hypothesis.
Talent selection programmes choose athletes for talent development pathways. Currently, the set of psychosocial variables that determine talent selection in youth Rugby Union are unknown, with the literature almost exclusively focusing on physiological variables. The purpose of this study was to use a novel machine learning approach to identify the physiological and psychosocial models that predict selection to a regional age-grade rugby union team. Age-grade club rugby players (n = 104; age, 15.47 ± 0.80; U16, n = 62; U18, n = 42) were assessed for physiological and psychosocial factors during regional talent selection days. Predictive models (selected vs. non-selected) were created for forwards, backs, and across all players using Bayesian machine learning. The generated physiological models correctly classified 67.55% of all players, 70.09% of forwards, and 62.50% of backs. Greater hand-grip strength, faster 10 m and 40 m sprint, and power were common features for selection. The generated psychosocial models correctly classified 62.26% of all players, 73.66% of forwards, and 60.42% of backs. Reduced burnout, reduced emotional exhaustion, and lower reduced sense of accomplishment, were common features for selection. Selection appears to be predominantly based on greater strength, speed, and power, as well as lower athlete burnout.
Directing our focus of attention appropriately during task execution can benefit outcome performance, cognitive efficiency, and physiological efficiency. For instance, individuals may benefit from adopting an external focus of attention (i.e., by focusing attention on the effects of one's movements on the environment) over an internal focus of attention (e.g., focusing on one's body movements). However, accounts concerning the theoretical functioning of such effects have primarily relied on hierarchical information processing perspectives; far less consideration has been given to potentially alternative explanations based on ecological dynamics, instances where an internal focus may be desirable over an external focus, and the associated applied implications. Within the present review, we: (a) outline the most recent developments in attentional focus research; (b) evaluate similarities and differences between information processing and ecological dynamics explanations of the focus of attention effect; (c) provide practical recommendations; and (d) discuss future research avenues. In doing so, a case is made for an “Ecological Dynamics Account of Attentional Focus” to act as an alternative to information processing-based hypotheses.
