AIM: to assess the effectiveness of micronized purified flavonoid fraction (MPFF) in conservative treatment for chronic hemorrhoids according to high-resolution anoscopy (HRA).PATIENTS AND METHODS: the study included 192 patients with chronic hemorrhoids, stage III. The general recommendation for all patients was dietary fibers for soft stools. The main group included 96 patients treated with MPFF for 2 months, 96 controls used dietary fibers only. The treatment effectiveness was evaluated due to high-resolution anoscopy data and clinical manifestation of the disease. The calculation of the drug intake compliance was carried out as well.RESULTS: patients of the main group showed significant clinical improvement after treatment in prolapse correction (66% vs 27%; p=0.001), in pain intensity decrease (5 times vs 1.5 times; p=0.03), in bleeding incidence (4 times vs 1.5 times; p=0.001). HRA showed significant reduction of inflammation (from type 2 to type 1) in main group in 50% and 20% in controls (p=0.02). Compliance with the MPFF in main group 1 was 74%, the total compliance of dietary fiber intake in both groups was 66.3%.CONCLUSIONS: combination of MPFF with dietary fiber intake significantly reduces clinical manifestations of chronic hemorrhoids stage III and HRA shows significant reduction of inflammation in hemorrhoid piles in these cases.
AIM. Improvement of treatment results in patients with the rectocele using of a two-level plastic of a pelvic bottom. METHODS. The plastic to a rectocele was carried out from the combined transvaginal and transrectal approach and consisted in sacrospinal-cardinal ligament, the colporrhaphy added transrectal «11 hours» with a mucopexy on height to 5 cm from the dental line. RESULTS. The remote results of treatment were tracked in terms from 12 months till 5 years after operation (a median of 3,5 years). In the studied group the «good» result was noted at 37 (75,5 %) patients, «satisfactory» and «unsatisfactory» - at 11 (22,4%) and 1 (2,0%) patients, respectively. Recurrence of a disease was taped at only 1 patient (2 %). CONCLUSION. The two-level pelvic plastic with the combined vaginal and transrectal access is associated with short operating time of 50 min., intraoperative blood loss about 150 ml and low perioperative morbidity.

Ростов-на-Дону В современной хирургии прямокишечных свищей, отмечается тенденция к большему использованию минимально инвазивных, сфинктеросохраняющих методик лечения. Однако немногочисленность имеющихся данных, требует проведения сравнительных исследований, а также более глубокого изучения эволюции патоморфологических изменений свищевого тракта в различные сроки заболевания. ЦЕЛЬ. Изучить эволюцию патоморфологических изменений свищевого тракта у больных с прямокишечными свищами. МАТЕРИАЛЫ И МЕТОДЫ. Первым этапом проведено исследование гистологического строения свищевого тракта у пациентов с различным сроком формирования прямокишечных свищей. Вторым этапом изучали гистологическую структуру свищевого тракта после лазерной коагуляции in vitro у больных с несформированными и длительно существующими сформированными прямокишечными свищами. РЕЗУЛЬТАТЫ. Данные гистологических исследований показали статистически значимые различия патоморфологических изменений свищевого тракта в зависимости от сроков заболевания. Применяемые режимы лазерного воздействия при сформированных свищах, по данным гистологических исследований, избирательно воздействовали на стенку свища и были достаточными для деконтаминации всего тракта, при этом, патоморфологические изменения не распространялись за пределы фиброзной капсулы. ВЫВОДЫ. Сроки статистически достоверного формирования фиброзной капсулы, по данным наших исследований, составили более 12 недель. Статистический анализ показал наличие достоверных различий в глубине патоморфологических изменений, вызванных лазерным излучением у больных со сформированными (более 8-12 недель) и несформированными (менее 6 недель) свищами. To assess pathomorphological changes in perianal fistulas to estimate rationale for FILaC procedure. PATIENTS AND METHODS. Cohort study included 76 patients, 42 (55.3%) females, aged 23-72 (median 42.7) years. Traditional fistulectomy was performed in 37 (48.7%) of them, 39 (51.3%) underwent FILaC procedure (laser coagulation of fistula track with excision of extrasphincteric part of fistula and preservation of intershincteric part). Histological study was performed in all cases with hematoxylin-eosin staining. RESULTS. Fistula morphological structure varied depending of duration of the disease. No fibrous capsule was detected in fistulas track up to 6 weeks, young connective tissue was found within 8-12 weeks and prominent fibrous capsule -after 12 weeks. A depth of laser coagulation thermal impact was 1.5±0.5 mm in cases with duration of disease >12 weeks, in cases <6 weeks -5.0±1.0 mm (p<0.0001). CONCLUSION.
