A case of reversible renal failure due to sarcoid granulomatous nephritis is described. The patient, a 21-year-old student, was admitted with renal insufficiency (GFR =25 ml/min); no damage was found in any organ except for slightly enlarged pulmonary hilar lymph nodes. Repeated percutaneous renal biopsies showed an interstitial noncaseating granulomatous nephritis. Steroid therapy provided rapid improvement and the diagnosis of sarcoidosis was established. Complete recovery of renal function with normal urinalyses was seen.
A prospective cross sectional study was conducted in the Department of Medicine, Christian Medical College and Hospital and Civil Hospital, Ludhiana. Patients between the age group 18-70 years were included in the study. 790 patients were taken for the study. Numbers of males were 65.4% and females 34.6%. In the study, it was found that in patients with 2 weeks cough duration, sputum AFB was positive at 2 weeks as well as 3 weeks. But in those patients who were negative at 2 weeks duration, when they were re-examined at 3rd week, the positivity of sputum AFB had increased which was statistically significant. So sputum analysis at 3 weeks is better than 2 weeks for tuberculosis evaluation, although evaluation of sputum should be done early at 2 weeks, cough duration for early diagnosis and treatment initiation.
A 53 year old lady was apparently well about 2 months back when she started complaining of painless subcutaneous swellings over the face and both the upper limbs.No systemic involvement was present .Histopathology of the lesion showed sarcoidosis.Patient responded to steroids .Serum ACE level was also raised(95). Subcutaneous nodules a rare presentation of sarcoidosis is discussed.
A 38 year old man was admitted to an emergency department with complains of fever for 2 days and bilateral upper and lower weakness. Dengue NSI came to be positive. Platelets were also found to be on the lower side. Serum potassium found to be on the lower side .power on the lower limb 3/5 and upper limb 4/5.power improved after potassium replacement.
