Objective: To demonstrate complications and difficulties that had been encountered during major gynaecological surgery.Materials & Methods: This retrospective hospital based study was carried out in the department of Obstetrics & Gynecology of Rajshahi Medical College & Hospital, Rajshahi, between 1st January 2002 to 30th june 2002.Result: Among 266 operated patients during the above mentioned 6 months period 100 samples were taken randomly. Regarding age distribution of the operated patients, 38% were in the age group of 31 to 40 years, more than ¼ th (33%) were in the age group of 41 to 50 years. Out of 100 patients about half (54%) of them were in the parous group of 1 - 5 children, next to this 28% had more than 5 and above babies. After sugery, 70 patients had no postoperative complications; only in 30 patients minor complications were noted. Among these, pyrexia above 100.0F was noted in 15 patients, lower abdominal pain in 8 patients and urinary tract infection in 5 patients.Conclusion: This study revealed a low rate of complications and difficulties encountered during major gynaecological surgery. In most cases due to highly skilled personnel, improvement of surgical technique, safe anaesthesia and appropriate use of antibiotics with blood transfusion, the mortality rate has remarkably decreased to nil.KYAMC Journal Vol. 5, No.-1, Jul 2014, Page 444-448
The study has been carried out to observe the effect of calcium therapy on urinary calcium/creatinine ratio in healthy gravid women and pre-eclamptic women. Calcium/creatinine ratio has been considered for early detection of pre-eclamptic toxaemia (PET). So for the diagnosis of PET we try to investigate relation of urinary calcium, urinary creatinine, urinary calcium/creatinine ratio before and after calcium therapy. For this purpose total number of 60 healthy gravid and clinically suspected PET women of 20- 28 weeks of pregnancy having age ranged from, 17-39 years were selected. Of these 30 healthy pregnant women were taken as control group and another clinically diagnosed 30 PET women were taken as observational group. Urinary calcium/creatinine ratio was within normal range in healthy pregnant women and after calcium therapy this ratio was not significantly changed. On the other hand urinary calcium/creatinine ratio in PET women was significantly decreased and this ratio was significantly increased after calcium therapy. These observations suggest that low calcium/creatinine ratio help in early prediction of PET and calcium supplement during pregnancy may reduce the incidence of PET. Hormonal changes and metabolic demands during pregnancy result in alteration in the physiological and biochemical parameters of thyroid function. Among the many physiological alterations which may occur during pregnancy are thyroid enlargement, increased thyroid capability for iodine uptake and increase in serum thyroxine and tri-iodothyronine concentration. However these findings are not usually associated with symptoms of hyperthyroidism. To clarify the changes, a study was undertaken in Rajshahi Medical College Hospital when serum specimens were collected from individual woman at different stages of normal pregnancy and women married but non pregnant. Among the study subjects, 22 women were in their 1st trimester, 20 women were in their 2nd trimester, 32 women were in their 3rd trimester of normal pregnancy and 10 women were married but non pregnant. We have measured serum levels of total and free thyroxine (TT4 & FT4) and total and free triiodothyronine (TT3 & FT3 ) by using RIA. During the 1st trimester of pregnancy serum FT4 and FT3 concentrations are significantly higher than in non pregnant women and women in 3rd trimester. The concentration of FT4 and FT3 progressively declined as the pregnancy advanced and are to control level in the 3rd trimester. The mean serum TT4 and TT3 are significantly elevated throughout pregnancy in comparison to control.KYAMC Journal Vol. 4, No.-2, Jan 2014, Page 415-418
