This study aimed to ascertain, for the first time, whether serum magnesium (Mg) concentration is affected by the presence of hepatocellular carcinoma (HCC). We retrospectively enrolled consecutive cirrhotic patients with a diagnosis of HCC (n = 130) or without subsequent evidence of HCC during surveillance (n = 161). Serum levels of Mg were significantly (P < 0.001) lower in patients with HCC than in those without (median [interquartile range]: 1.80 [1.62–1.90] mg/dl vs. 1.90 [1.72–2.08] mg/dl). On multivariate logistic regression, low serum Mg was associated with the presence of HCC (OR 0.047, 95% CI 0.015–0.164; P < 0.0001), independently from factors that can influence magnesaemia and HCC development. In a subset of 94 patients with HCC, a linear mixed effects model adjusted for confounders showed that serum Mg at diagnosis of HCC was lower than before diagnosis of the tumor (β = 0.117, 95% CI 0.039–0.194, P = 0.0035) and compared to after locoregional treatment of HCC (β = 0.079, 95% CI 0.010–0.149, P = 0.0259), with two thirds of patients experiencing these changes of serum Mg over time. We hypothesize that most HCCs, like other cancers, may be avid for Mg and behave like a Mg trap, disturbing the body’s Mg balance and resulting in lowering of serum Mg levels.
Background The aim of this study was to investigate how the COVID-19 pandemic influenced ERAS program application in colorectal surgery across hospitals in the Lazio region (central district in Italy) participating in the “Lazio Network” project. Methods A multi-institutional database was constructed. All patients included in this study underwent elective colorectal surgery for both malignant and benign disease between January 2019 and December 2020. Emergency procedures were excluded. The population was divided into 2 groups: a pre-COVID-19 group (PG) of patients operated on between February and December 2019 and a COVID-19 group (CG) of patients operated on between February and December 2020, during the first 2 waves of the pandemic in Italy. Results The groups included 622 patients in the PG and 615 in the CG treated in 8 hospitals of the network. The mean number of items applied was higher in the PG (65.6% vs. 56.6%, p < 0.001) in terms of preoperative items (64.2% vs. 50.7%, p < 0.001), intraoperative items (65.0% vs. 53.3%, p < 0.001), and postoperative items (68.8% vs. 63.2%, p < 0.001). Postoperative recovery was faster in the PG, with a shorter time to first flatus, first stool, autonomous mobilization and discharge (6.82 days vs. 7.43 days, p = 0.021). Postoperative complications, mortality and reoperations were similar among the groups. Conclusions The COVID-19 pandemic had a negative impact on the application of ERAS in the centers of the “Lazio Network” study group, with a reduction in adherence to the ERAS protocol in terms of preoperative, intraoperative and postoperative items. In addition, in the CG, the patients had worse postoperative outcomes with respect to recovery and discharge. (1) Background: We investigated, for the first time, whether dietary simple sugar intake affects MELD score changes over time in a cohort of cirrhotic liver transplant candidates. (2) Methods: the MELD score, dietary habits using a 3-day food diary, and visceral adipose tissue index (VATI) measured with CT scan were assessed in 80 consecutive outpatient cirrhotic patients at baseline, after counseling to follow current nutritional guidelines. The MELD score was reassessed after six months and the DELTA-MELD was calculated as the MELD at the second assessment minus the MELD at baseline. (3) Results: Compared with the baseline, the MELD score of cirrhotic patients at the end of the study was decreased, stable, or increased in 36%, 8% and 56% of patients, respectively. In separate multiple linear regression models, DELTA-MELD was positively and independently correlated with the daily intake of simple sugars expressed in g/kg body weight (p = 0.01) or as a percentage of total caloric intake (p = 0.0004) and with the number of daily portions of fruit, added sugar, jam, and honey (p = 0.003). These associations were present almost exclusively in patients with VATI above the median value. (4) Conclusions: In cirrhotic patients with high amounts of visceral adipose tissue the consumption of simple sugars and fructose should be limited to improve their clinical outcome.
BACKGROUND AND AIMS: Ulceration of the foot in diabetes is common and disabling Diabetic Foot Ulcers (DFU) are a dangerous and invalidating complication of diabetes. Traditionally they are classified by their aetiology, but many other factors may affect their evolution. Anamnestic, anthropometric and laboratory criteria are recommended for evaluation of Nutritional Status. They are suitable in clinical practice, but we argue that they are unfit to identify Malnutrition in specific populations, as DFU patients. Aim of our study was to evaluate the nutritional pattern and some other clinical and laboratory parameters among the ones tested routinarly in ambulatorial setting in a group of type 2 diabetic patients with DFU to characterize and single out the most efficient parameters useful for diagnosis of malnutrition. SUBJECTS AND METHODS: 23 non healing DFU subjects and 24 type 2 diabetic subjects without DFU, as control. For the evaluation of body composition we used bioelectrical impedance analysis (BIA 101 Akern r ). Phase Angle (pA) was pointed as a marker of extracellular/intracellular water rate and of body cell mass (BCM). We also recorded HbA1c, disease duration, Haemoglobin (Hb), Albumin, Total Proteins, Creatinin, C-Reactive Protein (CRP), BMI and any weight variation in the last two months. RESULTS: No significant difference for Albumin, Serum Proteins, CRP, Creatinin, Hb was found. Only the pA was significantly reduced (p = 0.001) in DFU subjects. Furthermore, the distribution of BMI and Hb values in reference ranges do not differ in DFU group and in the control group. CONCLUSIONS: The routine clinical and laboratory evaluation tests are not reliable to detect dynamic changes of the nutritional status in DFU subjects whereas bioelectrical measurements are useful in the evaluation of body cell mass changes in these subjects.
