The purpose of this study was to explore older adults' perceptions of participation in physical activity (PA) as it impacts productive ageing and informs occupational therapy (OT) practice. In this phenomenological study, 15 community-dwelling older adults were recruited using purposive and snowball sampling at community locations. Data collection methods included two interviews and an observation. The primary finding was that older adults continue individual patterns of meaningful PA across their lifespan when they have support to adapt to age-associated limitations, with a gradual decline in intensity during older years. Although this study's qualitative methodology limits broad generalizability, the findings provide applicability when situated in the context of community-living older adults interested in health maintenance through PA participation. OT practitioners have an important role with community-dwelling older adults to impact productive ageing by designing and promoting meaningful PA with adaptations that address unique, age-associated concerns. There is a need for further experimental research taking an occupational performance and health perspective to enhance the contribution of OT for this population's health-related quality of life through meaningful PA.
Senior population health often is underrepresented in curricula for medical and allied health students. Furthermore, entrenched and dense curricular schedules preclude interprofessional teams from clinical experiences related to senior population health. Community service learning potentially offers the opportunity to engage interprofessional students with a panel of older adults to assess health promotion metrics over time. To test this educational concept, we created Health Ambassador Teams for Seniors, also known as HATS. Utilizing a telehealth platform, interprofessional student teams were tasked with older adult wellness promotion. The annual Medicare wellness exam served as a template for patient encounters which was enhanced with key elements of geriatric assessment such as gait and balance, cognition, and functional evaluations. The objective was to have dyads of interprofessional students conduct telehealth visits and gather healthcare data to be used for serial patient encounters and track functional trajectories over time. As a proof of concept, pilot telehealth encounters with medical, physical therapy, nursing and occupational therapy students revealed that data on older adult functional performances such as gait speed, Timed Up and Go test (TUG), and Mini-Cog test could be acquired through telehealth. Equally importantly, trainees received diverse feedback from faculty, peers and volunteer patients. A Research Electronic Data Capture (REDCap) data repository allows trainees to track patient trends relative to their health promotion recommendations as well as handoff their patient panel to the next set of trainees. The HATS program promises to strengthen the Geriatric Workforce, especially with senior population health.
In the United States, the role of occupational therapy (OT) in provision of community-based health promotion is supported well in the literature; however, few practitioners are working in this arena. This mixed methods multiphase design study presents an example of a needs assessment process: assess before you assess. Participants included OT practitioners, residents in a low-income community housing complex, and older adult residents in another community housing complex. Methods included phenomenological and nonexperimental cross-sectional survey study design. Data analysis included coding, categorizing, and creating themes; composite scoring, Spearman correlations, and independent t tests for comparing variables. Results indicated that OT practitioners need more guidance and involvement to shift their paradigm from rehabilitation to community health promotion. Conducting needs assessments in three phases supports effective health promotion programming in community settings.
We present a 53-year-old female patient with median arcuate ligament syndrome (MALS), also known as Dunbar syndrome or celiac artery compression syndrome, related to lumbar lordosis and hip dysplasia. She utilized interprofessional management strategies, which were beneficial in reducing lumbar lordosis and MALS-related symptoms. This finding is important because there are no other reports in the literature describing interprofessional strategies to manage symptoms for patients who are waiting for surgery or are not candidates for surgery.
