Because of its limited reliability and the high prevalence of labral lesions, magnetic resonance arthrography provides a limited complementary benefit in the detection of labral lesions in patients with a high clinical suspicion of labral pathology. When there is a high clinical suspicion of a labral lesion, magnetic resonance arthrography has a poor negative predictive value and cannot be used to rule out a labral lesion. Physicians should critically consider whether the findings on a magnetic resonance arthrogram will alter the treatment strategy for an individual patient with a clinical suspicion of labral pathology.
Discussion persists about the outcome and results of hip arthroscopy in obese patients. Hip arthroscopy gained popularity over time. A current discussion is if obese patients can reach similar results after surgery compared with non-obese. To our knowledge, this is the first systematic review of literature about hip arthroscopy and obesity. We searched the Pubmed/Medline databases for literature and included three studies that compared the outcome of hip arthroscopy between different BMI groups. We extracted and pooled the data. For continues data a weighted mean difference was calculated, for dichotomous variables a weighted odds ratio (OR) was calculated using Review Software Manager. Heterogeneity of the included studies was calculated using I2 statistics. Data were extracted from two studies. In the Obese group, there was significant more conversion to total hip replacement or resurfacing hip replacement (OR = 2.21, 95% CI 1.07–4.56) and more re-arthroscopy (OR = 4.68, 95% CI 1.41–15.45). Any reoperation occurred more often in the obese group (OR = 2.87, 95% CI 1.53–5.38). In the Non Arthritic Hip Score obese scored lower than the non-Obese group [10.9 (−14,6 to 7.1)]. For the modified Harris Hip Score the score is − 6,6, according to the MCID this difference is clinically relevant. For both scores obese show lower outcomes but similar improvement after hip arthroscopy. Regarding a higher chance of needing a re-operation and lower subjective outcome scores obesity appears to have a negative influence on the outcome of hip arthroscopy.
Progressive ulnar translocation of the carpus and problems with the ulnar stump have been reported after resection of the distal ulna in rheumatoid arthritis (RA). However this has only occasionally been quantitatively assessed. In this study 24 wrists in 21 patients with RA were followed up for an average of 100 months after resection of the distal ulna. An additional 24 wrists in 14 non-operated RA patients were followed up for 92 months. In a retrospective radiographic analysis we demonstrate similar increases in ulnar translocation and ulnar-carpal distances in both groups.
Due to a lack of a validated Dutch version of the Hip Outcome Score (HOS) considering functional outcome after hip arthroscopy for femoroacetabular impingement syndrome, we validated the Dutch version of the HOS (HOS-NL) in patients with femoroacetabular impingement syndrome for reliability, internal consistency, construct- and content validity. Furthermore, the smallest detectable change (SDC) and minimal clinically important difference (MCID) were determined. All consecutive patients scheduled for an arthroscopic procedure for FAIS were selected. Five questionnaires covering groin and hip pain were filled in at three moments in time (two pre-operatively with a maximum two-week interval and 6 months postoperatively). Main endpoints were reliability (test re-test, SDC), internal consistency (Cronbach alpha), construct validity (construct validity was considered sufficient if a least 75% of a-priori made hypotheses were confirmed), content validity (floor and ceiling effects) and responsiveness (MCID). The intraclass correlation coefficient (ICC) was 0.86 for the HOS ADL-NL and 0.81 for the HOS Sports-NL. SDC for the HOS ADL-NL was 21 and for the HOS Sports-NL 29 Cronbach alpha score was 0.882 for HOS ADL-NL and 0.792 for HOS Sports-NL. Construct validity was considered sufficient since 91% of the hypotheses were confirmed. No floor effects were determined. A small ceiling effect was determined for the HOS AD-NL postoperatively. The MCID for HOS ADL-NL and HOS Sports-NL were 14 and 11.0, respectively. The HOS-NL is a reliable and valid patient reported outcome measure for measuring physical function and outcome in active and young patients with femoroacetabular impingement syndrome.
