Coexistence of pituitary adenoma (PA) and another type of brain tumor is a very rare clinical scenario. Even though such a presentation can be an incidental event but a thorough review of the literature will be made to elucidate the possible mechanisms and treatment options in similar cases. Two cases of concomitant sellar and suprasellar/diaphragmatic tumors are reported. A 37-year-old lady with prolactinoma and a suprasellar diaphragmatic meningioma and a 42-year-old acromegalic man with suprasellar/diaphragmatic meningioma and a PA. Both meningiomas were removed transcranially. The prolactinoma could be managed medically and the growth hormone secreting adenoma was removed trans-sphenoidally. The visual problems and hormonal imbalances of both patients improved postoperatively. The literature is reviewed on this topic and the possible pathogenesis and management protocol of similar lesions are discussed.
Real cost evaluation of water is necessary in agricultural products depending on obtained value by this input. In most areas of world especially in arid and semiarid areas, exist over pumping of ground water because the real value of water is much most than the costs of water supply and the lack of fit management water resources. In this study, using a sample of 110 farmers, water dealing value of over using of groundwater in Rafsanjan pistachio production area were investigated. Analysis and regression methods were used in this regard. The average determined value obtained 24 cents, for each share of water in this region which with over drafting of ground water, and decreasing quality and quantity of water has had significant relationship in the one percent significance level. Finally, for elimination or reduction of ground water degradation and its effects, this paper recommended in addition to reduction of licenses for ground water pumping. Determination of optimal economic water/land ratio in new and old pistachio producing areas is the other proposal of this research for alleviation groundwater over drafting effects. Permission for water conduction between wells and combination of fresh and saline water and also using desalination systems are methods for solving low quality of ground water.
As with other regions of Iran, due to excessive extraction of groundwater for intense agricultural activity, Ghorveh plain, a water-scarce irrigation district in the west of Iran, has faced a serious water crisis during the last decade. The present study investigates the impacts of two scenario policies, namely, non-price policy (as a supply-oriented policy) and water pricing policies (as a demand-oriented policy) on agricultural sector of Ghorveh Plain, using positive mathematical programming (PMP). The model was calibrated by using farm-level data for the crop years in 2016-2017. Our findings indicate that applying water supply constraint policy will change the land use and cropping pattern to the crops with higher water productivity. The increase of water resource constraints can lead to the increase of water economic return which indicates a rising value of water resources shortage, warning the producers of the agriculture sector to allocate water to the crops with higher economic value under the water resources shortage conditions. In addition, the findings underline that in a situation where the price of irrigation water is low due to the low elasticity of water demand in the agriculture sector, formulating the economic instruments such as rising water prices does not solely suffice to achieve sustainable water resource management. However, mixed scenarios emphasized that the water distribution policies should be aligned with the increases in water cost.
