BackgroundSepto-optic dysplasia, also referred to as de Morsier syndrome, is a congenital condition characterized by classic triad features: midline brain abnormalities, optic nerve hypoplasia and pituitary endocrine dysfunction. Sometimes, other various malformations appear within syndrome.Case presentationAn 11 and 1/2-year-old Caucasian Southeast European female patient with earlier established diagnoses of growth hormone deficiency, diabetes insipidus, seizures, mental retardation, optic nerve atrophy and right ptosis, was directed to us for consultative examination.The girl of short stature and low weight for her age had bilateral optic nerve hypoplasia, poor vision, nystagmus and right eye oculomotor palsy. Electroencephalogram revealed epileptic changes. Magnetic resonance imaging showed an empty sella syndrome, partial hypoplasia of corpus callosum, cavum of pellucid septum and diffuse polymicrogyria of the left temporal lobe. We found all elements of septo-optic dysplasia plus syndrome with right oculomotor nerve involvement.ConclusionBy earlier findings and evaluation, we established a diagnosis of septo-optic dysplasia plus. The case confirms the existence of various malformations within the syndrome and the need for the cooperation of several specialists in the diagnosis and treatment of children with the syndrome.

Intraosseous vascular malformation could be life-threatening due to uncontrolled hemorrhage after tooth extraction. According to biological behavior of this lesion, adequate diagnostic and treatment strategies are necessary in order to avoid possible complications. We reported cone beam computed tomography (CBCT) planning of an urgent en bloc resection of an intraosseous venous malformation by piezoelectric knife. Case report. A 55-year-old man was submitted to CBCT planning followed by piezoelectric knife resection of an intraosseous focal venous malformation of the mandibular body. Immediate reconstruction of the defect using iliac bone free graft was performed. The surgical treatment was uneventful and a 2-year follow-up revealed no signs of recurrence.Conclusion. Piezoelectric knife could provide precise, safe and bloodless procedure which is especially important in this pathology. Advantages of this technique are: lower risk of damaging soft tissue structures, precise osteotomy and bloodless surgery. Moreover, using piezosurgery bone knife, blood transfusion and blood transmitted diseases could be avoided. This case highlights the importance of CBCT as planning tool for resection of the mandible, using piezoelectric knife as safe method to achieve bloodless surgery.Nikolić Ž, et al. Introduction Spontaneous splenic rupture is a rare but potentially fatal complication of infectious mononucleosis (IM). It occurs in only 0.1-0.5% of cases of this disease. The aim of this paper was to present a case with spontaneous splenic rupture after IM. Case outline A 22-year-old female patient was feeling better one month after she was treated for infectious mononucleosis, and started training volleyball. Two weeks after starting the training, she felt severe abdominal pain. The diagnosis of rupture was confirmed with computer tomography. Splenectomy was successfully performed. The postoperative course was uneventful and the patient recovered with no need for blood transfusion. Conclusion Timely diagnosis and setting indications for surgical treatment are crucial in healing. Patients should wait to start with sport activities at least two months if the size of the spleen is within normal range.
Akutna krvarenja iz gornjeg dela gastrointestinalnog trakta su urgentna stanja sa velikim morbiditetom, i još uvek zna ajnim mortalitetom uprkos unapre enoj dijagnostici i terapiji. Cilj rada je ispitivanje determinanti težine krvarenja duodenalnih pepti kih ulkusa. Istraživanje je obuhvatilo 304 bolesnika hospitalizovanih zbog akutnog krvarenja iz gornjeg dela gastrointestinalnog trakta u petogodišnjem periodu. Le eni su u Klini ko-bolni kom centru Bežanijska Kosa u Beogradu. Dijagnoza je postavljena gastroduodenoskopijom. Od 304 bolesnika krvare i pepti ki ulkus imalo je 197/65% bolesnika. Krvare i duodenalni ulkus imalo je 144/73,1% bolesnika, sa naj eš om bulbarnom lokalizacijom 124/86,12%; 78/62,9% sa lezijom na zadnjem zidu bulbusa. 48/35,1% krvare ih duodenalnih ulkusa bilo je u Forrest Ib stadijumu; 68/47,2% imalo je veli inu ulkusne lezije 1,1-2,0 cm. Utvr ena je statisti ki zna ajna pozitivna korelacija izme u veli ine ulkusne duodenalne lezije i intenziteta krvarenja (p<0,005). Kod 68/79/86,1% bolesnika le enih endoskopski hemostaza je bila uspešna, pri emu se kod 13/19,1% dogodio recidiv krvarenja. Recidiv je kod 11/84,6% bio lokalizovan na zadnjem zidu bulbusa duodenuma.
