The aim of this study was to estimate time of biomineralization in developmental stages of rat lower incisors. Eruption length was measured. Four stages of incisor development were identified on histologic and microscopic computerized tomography (micro-CT) sections: (1) preodontoblast, (2) dentin matrix secretion, (3) enamel matrix secretion, and (4) enamel calcification. The overall eruption rate of the rat lower incisor was 600 ± 70 µm/day (mean ± SD; n = 12). The length of the enamel secretion was 4.59 ± 0.75 mm in histologic section, was 3.64 ± 0.63 mm in radiographic section, which converts to 180.4 ± 30.0 hours, 145 ± 25 hours respectively (n = 24). These findings suggested that the four biomineralizing developmental stages of the rat incisor took only several days. The significance of this animal study was to provide understanding for the rapid biomineralization process of developing rat tooth germ by analysis of tooth forming period.
Fibrous dysplasia is a developmental tumor-like condition that is characterized by replacement of normal bone by an excessive proliferation of cellular fibrous connective tissue intermixed with irregular bony trabeculae. Craniofacial lesions may cause facial pain, headache, cranial asymmetry, facial deformity, tooth displacement and visual or auditory impairment.In this case, a 2-year-9-month old boy who was diagnosed as the fibrous dysplasia showed delayed eruption on affected area. Teeth of left lateral dentition group have erupted completely but teeth of right lateral dentition group are erupting slowly. Eruption and maturation of affected teeth are in progress, so continuous observation is required presently. If the eruption state stops, surgical opening or forced eruption of the impacted teeth will be considered.
Supernumerary tooth occurs most frequently at premaxilla area. Followed by mandibular premolar area, mandibular fourth molar area, maxillary paramolar area. Mesiodens are mainly impacted in the palatal area and surgical approach is made at palatal side. The time of surgery remains controversial. In case of inverted or horizontal impacted supernumerary tooth, intraosseous tooth movement and vertical growth of premaxilla makes surgical extraction more difficult. And also the more quantity of removed bone is, the higher degree of difficulty is. Inverted mesiodens of these cases were impacted superior to apex level of adjacent permanent incisor. Although CT examination revealed exact location of impacted tooth, surgical procedure including ostectomy may take a long time more than expected. So, before surgical extraction, it' s need to be considered several factors such as necessity of CT taking, degree of difficulty, direction of surgical approach, necessity of general anesthesia etc.Key words : Deep Impaction, Mesiodens, Decision of Anesthesia of surgery, General Anesthesia, Local Anesthesia Abstract
