BackgroundAlternating wet and dry seasons may play an important role in the acquisition and distribution of Blastocystis subtype infection in the tropics. This cross-sectional study was therefore conducted to provide the prevalence of Blastocystis and to determine the potential risk factors associated with each subtype during the wet and dry seasons in the Aboriginal community, Pahang, Malaysia.MethodsA total of 473 faecal samples were collected: 256 (54.1%) and 217 (45.9%) samples were obtained during the wet (October-November 2014) and the dry season (June 2015), respectively. All fresh faecal samples were subjected to molecular analysis for subtype and allele identification.ResultsOf the 473 samples, 42.6% and 37.8% were positive for Blastocystis ST1, ST2, ST3 and ST4 during wet and dry seasons, respectively. Prevalence of Blastocystis ST1 was significantly higher during the wet season compared to the dry season (Z = 2.146, P < 0.05). Analysis of the association of each Blastocystis subtype with socioeconomic characteristics showed the presence of other family members infected with Blastocystis ST3 and the use of stored river water for domestic activities were the significant risk factors for Blastocystis ST3 infections during both seasons. Untreated water supply and low monthly household income (less or equal to RM 500) were the other significant risk factors for Blastocystis ST3 infections during wet and dry season, respectively. The presence of other family members with Blastocystis ST1 and ST2 was the only significant risk factor associated with ST1 and ST2 infections during both seasons. We hypothesise that transmission of Blastocystis ST1, ST2 and ST3 occurred from person to person during both seasons. The waterborne transmission was also identified as a mode of transmission of Blastocystis ST3.ConclusionThe significant risk factors identified in this study were important in the dynamic transmission of Blastocystis infections during both seasons. Blastocystis spp. is a unicellular enteric protozoan parasite in humans with a controversial role in disease etiology. It is common in developing countries among immunocompromised patients and people who have close contact with animals. In this study, we have systematically reviewed previous studies on the distribution and genotypes of human Blastocystis infection in Peninsular Malaysia. Studies examining the prevalence of Blastocystis in diverse demographics, including rural, urban, comorbid conditions, and high-risk populations, were taken into consideration. The infection has been reported in nine states; the total percentage of infection was 17.8% (1671/9397), with the most cases in Pahang (27.3%) and the least in Johor (3.4%). Molecular studies revealed the presence of six subtypes: ST1, ST2, ST3, ST4, ST5, and ST6. ST3 was reported as the predominant subtype in all the states, with a prevalence of 54.7% (338/618). The findings provide greater clarity on the epidemiology of Blastocystis in Malaysia, which will help in policy making towards planning and strategizing control measures against the parasite.
Background Understanding how environmental conditions can influence the insect distribution on corpses is essential to determine the postmortem interval. However, literatures related to forensically important flies in an aquatic environment were scarce. We report herewith a case series comprising three forensic cases related to floating corpses infested with fly larvae in Kuala Lumpur, Malaysia. Case presentation Case 1 involved the corpse of a 43-year old male found in the waterside of an abandoned mine. Case 2 was a 40-year-old male floating on a river stream, while case 3 was an 11-year-old boy who was found trapped in a monsoon drain after heavy rain. Numerous larvae of various stages, identified as Chrysomya megacephala (Calliphoridae) species, were collected in all cases even though the number of larvae was relatively lower compared to those usually collected on corpses found on terrestrial areas. Besides, the larvae of Eristalis spp. (Syrphidae), which are known to be exclusively associated with water bodies, were found in two of the cases. The blowfly Ch. rufifacies (Calliphoridae) larvae were collected only in case 3. Conclusions These findings show that both Ch. megacephala and Ch. rufifacies can be found in the ecologically varied death scene habitats including aquatic areas. Meanwhile, the presence of Eristalis spp. can be used as an indicator of death location associated with an aquatic environment. The challenges and limitations of analyzing entomological evidence on bodies discovered in aquatic environments were also discussed.
