A anquilose da articulação temporomandibular é a fusão entre o côndilo mandibular e a cavidade glenóide, gerando restrição aos movimentos articulares, função mastigatória limitada além de consequências estéticas e psicossociais. A condição ainda apresenta influência no desenvolvimento facial, erupção e posicionamento dentários quando ocorre em pacientes em crescimento. O estudo relata um caso clínico de AATM unilateral esquerda com infecção otológica prévia. A alteração evoluiu com completa impossibilidade de abertura bucal e complicações odontológicas e psicossociais. Artroplastia interposicional foi a técnica de escolha com utilização de retalho do músculo temporal como material de interposição acompanhada de coronoidectomia ipsilateral. Após o procedimento cirúrgico, obteve-se abertura bucal de 45 mm e o caso seguiu com tratamento multidisciplinar com fonoterapia e ortodontia. Dois anos após a cirurgia, o paciente apresentou abertura interincisal máxima de 37 mm e ausência de sinais de recidiva, o que confere estabilidade a conduta terapêutica proposta. This paper discusses issues about prison egresses and the process of social reinsertion of these individuals. Among the topics covered are discussions about the legal and institutional scenery of the prison egresses and reflections on the concept of the subject in the psychoanalytic context, as well as thoughts about the condition of the prison egresses and helplessness, according to Freud. The general objective of this paper consists in understanding the meaning of being an individual who is an egress of the prison system in the conception of the interviewed themselves. It is a qualitative research, in which depositions of seven collaborators, prison egresses, were considered, and the psychoanalytical method was used in the interpretation of the interviews. Considering the history of the people who passed through the prison system, that is, the pre-penal phase, the period of imprisonment, and the return to freedom, the meaning of being a prison egress pervades the need to deal with a concrete and objective reality hardly transformed, along with the aspects experienced during incarceration and all the subjective re-accommodations that the transition freedom-imprisonment-freedom imposes on the subjects, which encompasses the adaptation and misadaptation to the world inside and outside bars. The condition of the double helplessness of prison egresses leads to the fundamental helplessness along with the demands of dealing with the lack of guarantees of a better and more positive reality, as well as with expectationslittle optimistic and tied to a somber pastof breaking up with a life of criminality.
The Clinical Images Database (BIC) is an archive of clinical images for medical care and research. Objective: To describe the characteristics of and requirements for the creation of a BIC in a tertiary center in Northeastern Brazil. Methods: Technological advances allow for long-term storage of clinical images and retrieval according to clinical needs. With the patient consent and image anonymousness, the database can be used for research. It allows measurements/comparisons, reconstruction/ deconstruction and analysis of anatomical, clinical and pathological variables. The specific infrastructure and image capture and storage requirements for a BIC used for clinical and research purposes are analyzed. Results: Image capture and storage: Comprises centralization of X-ray, CT, MRI and angiography data through the network to a server. An anonymous version of each set of studies must be available for use in research. External backup is needed. Visualization: Comprises the user interface and includes stations for viewing and data processing. Maintenance: A technical team assists users and manages the system, troubleshooting problems, registering users and performing regular audits. An external committee supervised the creation of the BIC, and continues to review its progress. Conclusions: The present study could serve as a basis for the creation of similar clinical databases for research use
