Objective: The aim of the present study was to analyze potential drug interactions and adverse reactions to NSAIDs in elderly users of a private drug distribution service. Method: A prospective, exploratory and descriptive study with a quantitative approach was performed. The elderly users of NSAIDs attended by the service were interviewed and their prescriptions analyzed between May and September, 2014. Analysis of drug interactions was performed through computerized databases. The post-sales analysis of adverse reactions was performed using the Adverse Drug Reaction Probability Scale. Statistical analysis was performed with the Chi-squared and Fisher's Exact tests. Results: The study evaluated 200 elderly persons, among whom women predominated (56.5%). The average age was 65 years ±10. The NSAIDs accounted for 38.7% of prescription drugs used, and included dipyrone (26.9%), nimesulide (22.8%) and ketoprofen (16.3%). A total of 8.5% of such drugs were considered inappropriate medications for the elderly. A total of 104 potential drug interactions were identified, of which 24% were considered highly clinically significant. The NSAIDs with the greatest risk of interactions were ketoprofen 46.2%, ketorolac 14.4%, nimesulide 12.5% and diclofenac 9.6%. In post-sales monitoring 30.5% of the elderly persons reported undesirable symptoms after the use of NSAIDs, with stomach discomfort the most prevalent (17%). Conclusion: The present study confirmed the importance of monitoring the use of NSAIDs among the elderly due to the increased risk of drug interactions and adverse reactions associated with age, concomitant diseases, multi-prescriptions and polypharmacy. The choice of appropriate drugs for the elderly, the reconciliation of all the medications taken by the patient, and effective pharmaceutical care are measures that can contribute to the rational and safe use of NSAIDs.
Objetivou-se determinar a taxa de interações medicamentosas potenciais teóricas em prescrições para idosos diagnosticados com Síndrome Coronariana Aguda em um hospital de ensino. Estudo descritivo exploratório com análise de 607 prescrições através de bases de dados informatizadas para identificar e classificar as interações de acordo com a intensidade (maior, moderada ou menor), mecanismo (farmacocinético ou farmacodinâmico) e relevância da documentação. Foram detectadas 10.162 interações, distribuídas entre 554 tipos de combinações diferentes dentre os medicamentos prescritos, e 99% das prescrições apresentaram pelo menos uma e máximo de 53 interações, destacando-se a prevalência de maiores e moderadas. Houve correlação entre o número de interações e o número de medicamentos prescritos e o tempo de hospitalização. Este estudo contribui para a delimitação do padrão de prevalência das interações medicamentosas em prescrições para Síndrome Coronariana Aguda, além de subsidiar a importância da implantação efetiva da Farmácia Clínica em hospitais de ensino.
Several biosimilar versions of enoxaparin are already approved and in use globally. Analytical characterization can establish good quality control in manufacturing, but they may not assure similarity in clinical outcomes between biosimilar and branded enoxaparin. This study evaluated the efficacy and safety of biosimilar Cristália versus branded Sanofi enoxaparin in venous thromboembolism (VTE) prevention in patients undergoing major abdominal surgery at risk for VTE. In this randomized, prospective single-blind study, we compared Cristália enoxaparin (Ce), a biosimilar version, versus branded Sanofi enoxaparin (Se; at a dose of 40 mg subcutaneously per day postoperatively from 7 to 10 days) in 243 patients submitted to major abdominal surgery at risk for VTE for VTE prevention. The primary efficacy outcome was occurrence of VTE or death related to VTE. The principal safety outcomes were a combination of major bleeding and clinically relevant non-major bleeding. Bilateral duplex scanning of the legs was performed from days 10 to 14, and follow-ups were performed up to 60 days after surgery. The incidence of VTE was 4.9% in the Cristália group and 1.1% in the Sanofi group (absolute risk difference = 3.80%, 95% confidence interval [CI]: -1.4%-9.0%) yielding noninferiority since the 95% CI does not reach the prespecified value Δ = 20%. Clinically significant bleeding occurred in 9.9% in the Cristália group and in 5.5% in the Sanofi group (n.s. ). In conclusion, this study suggests that 40 mg once daily of Ce, a biosimilar enoxaparin, is as effective and safe as the branded Sanofi enoxaparin in the prophylaxis of VTE in patients submitted to major abdominal surgery at risk for VTE.
Introdução:As interações entre fármacos e nutrientes acarretam reações adversas e/ou ineficácia da farmacoterapia, além de provocar prejuízos no estado nutricional, sobretudo em idosos. Objetivo: Descrever a taxa de interações potenciais teóricas entre nutrientes da dieta e fármacos prescritos para idosos hospitalizados por Síndrome Coronariana Aguda em um hospital de ensino. Material e Métodos: Trata-se de um estudo transversal com delineamento descritivo e abordagem qualitativa que analisou 607 prescrições no período entre abril e julho de 2016, utilizando-se as bases de dados informatizadas Micromedex e Drugs.com. Resultados: Foram identificadas 4.313 interações distribuídas em 61 tipos entre os fármacos prescritos e nutrientes. Conclusão: Verifica-se alta taxa de interações entre fármacos e nutrientes nas prescrições para idosos com Síndrome Coronariana Aguda.
