Background: Many variations of ovariohysterectomy techniques have been described, including the traditional one and minimally invasive procedures. Non-laparoscopic Snook hook technique is an alternative for performing minimally invasive ovariohysterectomy. Few studies have been carried out in order to assess pain in animals submitted to minimally invasive surgeries, especially involving one of the most performed surgical procedures in veterinary practice. The aim of this study was to evaluate surgical duration, intraoperative nociception and acute postoperative pain after traditional ovariohysterectomy or minimally invasive non-laparoscopic technique in dogs using Snook hook. The hypothesis is that non-laparoscopic minimally invasive ovariohysterectomy would be faster and less painful than the conventional technique.Material, Methods & Results: Thirty dogs were divided into Traditional Group (TG = 15) and Minimally Invasive Group (MIG = 15). Heart rate, respiratory rate, systolic blood pressure, body temperature, oxyhemoglobin saturation, end-tidal carbon dioxide concentration (ETCO2) and end-tidal isoflurane concentration were evaluated before the surgery to start (M0), during incision (M1), clamping of the first ovarian pedicle (M2), second ovarian pedicle (M3), uterine cervix (M4), abdominal suture (M5) and at the end of surgery (M6). The modified Glasgow Pain Scale was used for acute postoperative pain assessment and Visual Analogue Scale (VAS) was used to assess the sensitivity of surgical wound. The level of significance established for all statistical analyzes was 5%. Statistical differences were not observed between groups considering total surgical time and postoperative acute pain intensity (P > 0.05), in spite of MIG having shorter duration of surgery. There was no statistical difference between groups considering all intraoperative parameters except respiratory rate (TG < MIG; P < 0.05) and ETCO2 (MIG < TG; P < 0.05) at the moment of traction of the first ovarian pedicle (M2). Pain assessment by VAS showed statistical difference 24h after the end of surgery (TG < MIG) (P < 0.05). Discussion: Both procedures were similar regarding intraoperative nociception and acute postoperative pain. It is possible that the sensation of pain in both procedures was blocked by the effectiveness of analgesics, once they might cause an inhibition of painful behaviors limiting a possible difference in pain identification. Higher respiratory stress observed in MIG at M2 and higher pain score by VAS noted in MIG 24 h after the end of surgery can be justified by greater traction of ovarian pedicle, due to limited surgical access of minimally invasive technique. Minimally invasive ovariohysterectomy non-laparoscopic seems to be potentially faster, probably due to the smaller size of the abdominal incision, which takes less time to be closed. In the present study, both techniques were performed by an experienced surgeon, providing safe procedures, nevertheless it is important to emphasize that iatrogenic injury can be caused by surgeons not proficient in the Snook hook technique, considering the limited visualization of anatomical abdominal structures. Data obtained indicate that traditional ovariohysterectomy and non-laparoscopic Snook hook technique promote similar intraoperative nociception and acute postoperative pain, however minimally invasive procedure is potentially faster and with less surgical trauma.
RESUMO Foi atendida, no Hospital Veterinário da Universidade de Cuiabá, uma coruja suindara (Tyto furcata), resgatada pela Polícia Militar Ambiental, sem histórico, adulta e 400 gramas de peso corporal. A ave apresentava impotência funcional do membro pélvico direito, além de desvio, crepitação e dor na região proximal tibiotársica direita, sem lesão cutânea. Estudo radiográfico revelou fratura diafisária oblíqua curta em tibiotarso direito. A estabilização da fratura foi realizada com placa bloqueada do sistema de 1,5mm, na função de apoio, seis orifícios, com três parafusos monocorticais distais, dois parafusos monocorticais proximais e um parafuso bicortical proximal. Ao 30º dia do pós-operatório, os achados radiográficos de consolidação óssea e o retorno da função do membro permitiram a alta médica. Concluiu-se que o uso de placa bloqueada em função apoio para estabilização de fraturas tibiotársicas simples de coruja suindara oferece resultados satisfatórios.
Vehicle collisions involving giant anteaters contribute significantly to the decline of this population. Although mortality rates at the time of trauma are high, many animals survive despite severe trauma and limb fractures. Treating these individuals is extremely challenging. This report describes the use of a corrective osteotomy to treat an angular deformity caused by inadequate healing of a tibial and fibular fracture in a young giant anteater. The animal was rescued on a highway by the Brazilian Institute for the Environment and Renewable Natural Resources (Ibama). He was presented at the University Veterinary Hospital with lameness of the left pelvic limb and edema in the middle region of the tibial shaft. The radiographic examination showed an exuberant callus and significant bone deviation in the middle third of the left tibia and fibula. Corrective wedge osteotomy and fixation with a 3.5mm locking plate and 2.5mm intramedullary pin were performed. In the immediate postoperative period, there was functional recovery of the limb and complete consolidation of the osteotomy was identified 60 days after the operation. The corrective wedge osteotomy technique was effective for the treatment of inadequate tibial consolidation in a young giant anteater, providing complete recovery of all limb functions.
Introduction The aim of this study was to describe and evaluate a novel plate designed specifically for the canine ilium using finite element methods. The cranial portion of the plate had an elliptical shape and contained nine screw holes. The caudal portion of the plate was triangular with three screw holes. Four and three screws were used at the cranial (screw 1 to 4) and caudal (screw 5 to 7) segment of the plate. Finite element models of the plate and canine pelvis were created. A load of 300 N was applied on the femur-hip joint contact area. Values of Von Mises Stress on the plate, screws and the maximum and minimum main stresses in the bone were used to capture the mechanical factors in this study. Results The novel implant had a plate stress of 51.9 megapascals (MPa) with higher stress in the dorsal part of the plate. Screws 2 and 4 showed similar stress values of 17.3 MPa. Screws 1 and 3 were the most loaded (51.9 MPa and 75 MPa, respectively). Screws 5, 6, and 7 showed similar dissipation and stress values (21.55 MPa). There was traction force in the dorsal region of the pubis and compression in the ventral part, with dissipation and values of 15.4 MPa and 23.9 MPa, respectively, acquiring balance between them. Conclusion The novel plate is potentially applicable and specifically suitable for iliac fractures in dogs. The screws distribution modeled proved itself strategical since the simulated stresses were evenly distributed throughout the implant construct.
