Leech infestations can lead to several symptoms, including epistaxis, hemoptysis, hematemesis, vaginal bleeding and anemia. A five-year-old male patient presented to the emergency ward with bilateral bleeding from the nose in the form of leakage with a duration of one month, as well as bloody vomiting and blood in the sputum for two days prior to admission. The patient was pale, the nose and oropharyngeal mucosa were pale with fresh leakagetype bleeding and there was a similar clot in the nasopharynx. Laboratory examinations were normal. Examination of the oropharynx and nasopharynx was repeated to assess for active bleeding, and a leech 4x1 cm in size was found in addition to a clot. The leech was detached from the mucosa following administration of a hypertonic saline solution and removed with forceps. Leech infestation should be considered in cases of bleeding and anemia of unknown etiology in patients from rural areas.
Amaç: Bu galifmanin amaci dürtüsellik ve emosyonel faktörlerin obezite ile olan ili §kisini incelemektlr. Yöntem 1er: Çaliçmaya dâhil edilen toplam 48 obez birey, 48 bireyden olu §an sa^likh kiloya sahip kontrol grubuyia kiyaslanmi;tir. Katilimcilara DSM-IV öiceklerine göre yapilandirilmi; klinik görü;me formu (SCID-I), Yeme Yutumu testi (YTT), Beck depresyon ölgegi (BDÖ), Barrat dürtüsellik ölcegi-11(BlS-l 1 ) uygulandi. Bulgular: Çali §maya dâhil edilen 48 obez birey tikmircasina yeme bozuklugu olanlar ve olmayanlar olarak iki alt gruba aynldi. Tikmircasina yeme bozuklugu olan 22 ki;i (19 kadin, 3 erkek), tikmircasina yeme bozuklugu olmayan 26 ki;iydi (25 kadin 1 erkek). Dürtüsellik puanlarinda gruplar arasinda bir fark saptanamadi (p>0,05). übez olan 48 kiçinin 25'ine DSM-IV tani ölcütlerine göre depresif bozukluk tanisi kondu. Kontrol grubundaki 48 kifiden 8'ine depresif bozukluk tanisi kondu. Yeme tutum testi ve Beck depresyon öicek puanlan obez grupta kontrol grubuna göre istatistiksel olarak anlamli oranda yüksektl (p<0,05). Depresyonu olan ve olmayan gruplarda dürtüsellik puanlan kar;ila;tirildi. Depresyon olan grupta dürtüsellik puanlan depresyonu olmayanlara göre anlamli oranda yüksek bulundu (p<0,05). Benzer ;ekilde obez bireylerin oldugu grup da depresyon varh^ma göre ayrildi^inda depresyonu olanlann dürtüsellik puanlan di^er gruba göre anlamli olarak yüksek saptandi (p<0,05). Sonuç: Bu çalijmada one çikan bulgu obezltenin emosyonel özelliklerle ili;kisinin dürtüsel özelliklerle ili;kisinden daha güclü oldu^udur. Çali;mada ayrica depresyonu olanlann dürtüsellik puanlan da yüksek olarak saptanmijtir. Bu sonuç obezite ve dürtüsellik ili;kisinden çok depresyon ve dürtüseilik ili;kisine baglanmi §tir.
