Heat waves are one of the most common direct impacts of anthropogenic climate change and excess mortality their most apparent impact. While Turkey has experienced an increase in heat wave episodes between 1971 and 2016, no epidemiological studies have examined their potential impacts on public health so far. In this study excess mortality in Istanbul attributable to extreme heat wave episodes between 2013 and 2017 is presented. Total excess deaths were calculated using mortality rates across different categories, including age, sex, and cause of death. The analysis shows that three extreme heat waves in the summer months of 2015, 2016, and 2017, which covered 14 days in total, significantly increased the mortality rate and caused 419 excess deaths in 23 days of exposure. As climate simulations show that Turkey is one of the most vulnerable countries in the Europe region to the increased intensity of heat waves until the end of the 21st century, further studies about increased mortality and morbidity risks due to heat waves in Istanbul and other cities, as well as intervention studies, are necessary.
The contours of Turkey's climate policy have remained almost intact over the past two decades. Being an Annex I party without any mitigation commitments, Turkey maintains a peculiar position under UNFCCC. Subsequent to 12 years of delay in signing both the Framework Convention and the Kyoto Protocol, Turkey had the highest rate of increase in greenhouse gas emissions among the Annex I countries with 110.4% upsurge in the period 1990 and 2013. Yet with the new climate regime now in place, the country's mitigation pledges fall short of expectations both in terms of realistic projections and its ambition to step up in the post‐2020 period. Climate policies in Turkey, an EU candidate and OECD founding member with a growing economy, remain under‐investigated. Although the country has a wide range of policies and institutions in place, it shows limited progress in addressing climate change. Based on evidence from the literature, we observe that climate policies operationalize in Turkey insofar as they do not directly confront developmental ambitions, leaving policy diffusion with limited success. To provide a historic overview, we focus on climate policy development, actors, processes, and contemporary trends. Evidence shows that these are highly ridden with the politics of special circumstances: a notion that Turkey employs to refrain from bindings commitments. In order to go beyond special circumstances discourse, we argue the need for a bold policy shift in Turkey, a country subject to adverse impacts of climate change and high‐carbon lock‐in risk due to development policy preferences. OBJECTIVE: The aim of this study was to examine the relationship between executive functions and suicidality in terms of impulsivity and depression severity. METHODS: Depressed adolescents who have made a suicide attempt in the last year (n = 32), depressed adolescents without a suicide attempt (n = 30), and healthy controls (n = 30) participated in the study in Dokuz Eylul University Faculty of Medicine, Department of Child and Adolescent Psychiatry. Clinical diagnoses were made according to the DSM-IV by applying Kiddie Schedule for Affective Disorders and Schizophrenia Present and Lifetime Version (K-SADS-PL). Data were collected by using a sociodemographic data form, the Beck Depression Inventory (BDI), Barratt Impulsiveness Scale, and the Children's Depression Rating Scale-Revised (CDRS-R). To evaluate all participants' intelligence scores, the Wechsler Intelligence Scale for Children (WISC-R) was applied for adolescents aged under 16 years, and the Wechsler Adult Intelligence Scale (WAIS) was applied to adolescents aged 16-18 years. To determine the performance-based executive functions, Wisconsin Card Sorting Test and Stroop Test were applied to all participants. RESULTS: In this study, the participants who had made a suicide attempt displayed lower performance in the Stroop Test, especially in part 1 and part 4 compared with the controls (p = .04 and p = .011). Depressive patients also exhibited lower performance in the Stroop Test in part 3 compared with the controls (p = .049). Impulsivity was found more severe in depressive patients compared with controls (p < .001). There were no statistically significant differences between depressive patients with or without suicide attempt in terms of depression severity. CONCLUSION: Executive dysfunction appears to be associated with suicidal behaviour in adolescents with the major depressive disorder. This findings need to be replicated with a larger sample size in the future.
The case of Turkey provides some insight into the socio-political and communicative processes taking place at the periphery of global climate governance efforts. Turkey’s 12-year delayed entry into the United Nations Framework Convention on Climate Change regime (in 2004) and its being one of the last signatories to the Kyoto Protocol (in 2009) has hampered climate-relevant efforts in the country in many ways. This includes institutionalization at national and local levels, the development of relevant national policies, and communication activities. Climate change communication activities in Turkey can be divided into two major categories: the earlier advocacy activities, and the period of mass communication. The earlier activist or advocacy group communication efforts began around 2000, and have contributed significantly to mainstreaming climate change. Paralleling the government’s position towards the issue in many ways, the national-level media activities have remained nominal until 2007, when escalating local weather extremes were widely associated with climate change. Research in climate change communication in Turkey commenced only recently. Although the studies are limited both in scope and quantity, existing evidence suggests that 2007 was crucial in setting the terms of the debate in the country. Mobilizations at both international and national levels in 2009 made that year another landmark for climate change communication and policy in Turkey. International organizations and governance agencies have also taken active roles in both communication and research activities, and in the translation of governance tools developed at the international level to the national level. A review of the above-mentioned efforts suggests that a bottom-up direction of climate change communication efforts, and a minority-influence framework—in which minor advocacy and expert groups are supported by global policy norms and scientific knowledge in taking the issue to the national agenda—may be useful in understanding the dynamics taking place in industrializing countries such as Turkey.
Climate change is one of the biggest health threats facing humanity and can directly affect human health through heat waves. This study aims to evaluate excess deaths during heat waves between the summer months of 2004 and 2017 in Istanbul and to determine a definition of heat waves that can be used in the development of an early warning system, a part of prospective urban heat-health action plans. In this study, heat waves were determined using the Excess Heat Factor, an index based on a three-day-averaged daily mean temperature. The death rates during heat waves and non-heat wave days of the summer months were compared with a Z test of the difference of natural logarithms. Thirty heat waves were recorded in Istanbul during the summer months of 2004–2017. In 67% of the heat waves, the death rate was significantly higher than the reference period and 4281 excess deaths were recorded. The mortality risk was especially higher during heat waves of higher intensity. The study showed an excess risk of mortality during heat waves in Istanbul, and the findings suggest that the Excess Heat Factor could be an appropriate tool for an early warning system in Istanbul.
