Aim: The purpose of this in vitro study was to evaluate the effect of different layering techniques (bulk, oblique, centripetal, split horizontal) on the marginal microleakage of Class II adhesive restorations. Methodology: Forty-eight standardised Class II cavities were prepared on both mesial and distal sides of extracted non-carious human molar teeth. Following preparation, all specimens were randomly divided into four groups (n=12). Group 1: bulk technique; Group 2: oblique incremental technique; Group 3: centripetal incremental technique; Group 4: split horizontal incremental technique. The teeth were restored with a total-etch adhesive system and a microhybrid composite resin. After thermocycling and immersion in a 0.2% methylene blue solution for 24 hours, the restorations were sectioned, and dye penetration was evaluated under a stereomicroscope. The microleakage scores (0 to 4) obtained from the occlusal and gingival margins were analysed with Kruskal-Wallis and Mann-Whitney U tests (p<0,05). Results: Incremental placement techniques showed lower microleakage compared with bulk and lower microleakage was seen at occlusal margin compared with gingival margin. However, there were no statistically significant differences among four placement techniques and the margins (p>0,05). Conclusion: Based on the results of the present study, none of the four placement techniques were found to be effective in eliminating marginal microleakage entirely in Class II restorations. All of the placement techniques showed similar results when the cavity margins were located on the enamel.
ÖZAmaç: Bu olgu sunumunun amacı; komplike kron kök kırığı bulunan maksiller kesici dişlerin reataçman tekniği kullanılarak yapılan multidisipliner tedavisini ve 12 aylık takibini sunmaktır.Olgu Sunumu: Yirmi bir yaşındaki erkek hasta maksiller orta keser dişleri ve sol yan keser dişinde 2 gün önce kaza sonucu oluşan kırık ve ağrı şikayeti ile kliniğimize başvurdu. Hastanın herhangi bir sistemik hastalığı olmadığı tespit edildi. Klinik ve radyografik muayenede maksiller orta keser ve sol yan keser dişlerin kronlarının orta üçlüden kırıldığı belirlendi. Hastanın dişlerinin kırık parçalarını muhafaza ettiği belirlendi. Ilgili dişlerin lastik örtü ile izolasyonunun ardından konservatif endodontik giriş kaviteleri hazırlandı. %5,25 NaOCl irrigasyonu altında dişlerin kök kanalları ProTaper Next NiTi döner eğe sistemi ile X4 (40/.06)' e kadar genişletildi. Daha sonra kök kanalları gutta-perka ve kök kanal patı ile lateral kompaksiyon tekniğine uygun olarak dolduruldu. Ertesi gün yapılan seansta kırık dişler kendilerine ait kırık parçalar ile Panavia F 2.0 rezin siman kullanılarak restore edildi. On iki ay sonra yapılan klinik ve radyografik muayenede ilgili dişlerin asemptomatik olduğu ve periapikal dokuların sağlıklı olduğu tespit edildi.Sonuç: Komplike kron kırık vakalarında, kırık dişlerin kendine ait parçalar ile restore edilmesi iyi bir tedavi alternatifi olarak değerlendirilebilir.Anahtar sözcükler: Komplike Kron Kırığı, Travma, Reataçman Tekniği, Endodonti, Konservatif Tedavi. ABSTRACT Aim:The aim the present case report was to describe the multidisciplinary treatment procedure of maxillary incisors with complicated crown fracture using reattachment technique and 12-months follow-up.Case Report: A 21-year-old male patient applied to our clinic with the chief complaint of fractured upper central incisors and upper left lateral incisor due to an accident 2 days before. Patient's medical history was noncontributory. Clinical and radiographic examination revealed complicated horizontal crown fractures in the middle third of the crowns of maxillary incisors. The patient also presented the fragment of the detached teeth that had broken. After isolation with rubber dam, conservative endodontic access cavities were prepared. Under copious irrigation with 5.25% NaOCl, teeth were instrumented up to X4 (40/.06) with ProTaper Next NiTi rotary instruments. Root canals were obturated with gutta-percha and sealer using lateral compaction technique.
