Plasma medicine is a dynamically developing field of science based on the physical and chemical properties of air-plasma flows. Low-temperature plasma is a partially ionized gas with a temperature of about 40C, the synergistic interaction of its active substances is a successful method of sterilization, decontamination of wound surfaces, treatment of surgical and burn wounds. Due to the combined action, over the past decades, low-temperature plasma has proven itself in purulent surgery, dermatology, and dentistry, but in addition, it has opened up great prospects in oncology. Possessing the effects of inhibiting inflammatory processes, stimulating regeneration processes, improving microcirculation and tissue metabolism, plasma flows are also used in medical rehabilitation. The review considers the possibilities of using low-temperature plasma in various fields of medicine, as well as the effect of plasma flows on basic cellular processes and various forms of cell death. The experience of using low-temperature argon plasma in the complex rehabilitation of patients with malignant neoplasms is presented. Literature sources from Russian and foreign databases (Elibrary, PubMed) on the topic of low-temperature plasma application in medical practice were analyzed. The analysis showed that plasma medicine is a rapidly developing modern direction, opening up new opportunities in healthcare. Further studies of plasma streams are needed to unlock their full therapeutic potential and fully understand the mechanisms of action.
Taking into account the high prevalence of complications after radical surgical treatment of breast cancer, medical rehabilitation courses were developed and conducted in the early postoperative period (2–4 days after surgery) with the inclusion of physical factors: low-temperature argon plasma and general magnetotherapy, as well as physical therapy, biofeedback training on the support reaction (balance therapy) and individual classes with a medical psychologist. The study included 71 women who underwent surgery for breast cancer on 2–4 days after the surgery. The patients were divided into 2 groups, comparable in age, clinical and functional indicators, by simple randomization. At all stages of the study, the patients underwent clinical, functional and laboratory examinations, and questionnaires. In group1 (n = 36), 10 procedures of low-temperature argon plasma and general magnetotherapy were carried out in the early postoperative period against the background of standard therapy. In the 2nd group (n = 35,) the patients received low-temperature argon plasma. In addition to physiotherapy, all patients received a complex of physiotherapy exercises, balance therapy and classes from a medical psychologist. The results showed the disappearance of puffiness in the postoperative area and the upper limb, an increase in the volume and quality of movements, reduction in the manifestation of pain syndrome, as well as inflammation syndrome, acceleration of regeneration, prevention of the formation of rough scar tissue. The patients noted an improvement in general health, improved sleep. With further observation, the prolongation of the action of physical factors was noted. Thus, the combined effect of the studied factors makes it possible to prevent both early and late complications of the postoperative period and makes a positive effect on the restoration and maintenance of the quality of life, and the social adaptation of breast cancer patients.
Применение низкотемпературной плазмы -современное развивающееся направление в медицине, в том числе в реабилитации. Низкотемпературная плазма обладает высоким антисептическим ранозаживляющим эффектом, а также является безопасным методом физического воздействия. В условиях стационара Клиники имени профессора Ю.Н. Касаткина Российской медицинской академии непрерывного профессионального образования Минздрава России было проведено лечение пациента с использованием низкотемпературной аргоновой плазмы после селективной шейной лимфодиссекции справа, резекции нижней челюсти, тканей дна полости рта с пластикой дефекта малоберцовым аутотрансплантатом по поводу рака слизистой десны нижней челюсти справа pT4aN0M0 IVa стадии. В процессе дистанционной лучевой терапии у пациента развилась местная лучевая реакция с образованием свищевого отверстия в области послеоперационного рубца. Свищевое отверстие и пораженную область обрабатывали охлаждённым воздушно-плазменным потоком. На фоне продолжающегося применения низкотемпературной плазмы были отмечены прекращение гноетечения, уменьшение гиперемии и отёка в области повреждённых ионизирующим излучением тканей. Пациенту была своевременно продолжена лучевая терапия.Показано, что использование данного лечебного фактора в программе медицинской реабилитации является эффективным способом локального воздействия на патологический очаг, позволяет добиться быстрого репаративного эффекта, устраняет и предотвращает развитие бактериальных осложнений. Кроме того, применение воздушно-плазменного потока способствует быстрому купированию проявлений лучевых реакций, оказывает благоприятное влияние на повреждённые ткани, а также сокращает срок реабилитации тяжёлых онкологических больных в постоперационном периоде и в процессе проведения курса дистанционной лучевой терапии. Результаты действия низкотемпературной плазмы положительно влияют на общее состояние пациентов и улучшают качество их жизни.Ключевые слова: медицинская реабилитация; низкотемпературная аргоновая плазма; осложнение хирургического лечения; лучевая терапия; лучевая реакция; клинический случай.
BACKGROUND: The problem of medical rehabilitation/habilitation of patients with diseases of the nervous system, mental disorders, congenital anomalies and developmental delays is extremely relevant. The article analyzes the comprehensive care for this category of patients on the basis of the State Budgetary Institution Center for Comprehensive Rehabilitation of the Disabled "Butovo". AIM: analysis of the volume and methodology of prescribing physical factors in the provision of medical and social services to patients at the State Budgetary Institution Center for Comprehensive Rehabilitation of the Disabled "Butovo". MATERIALS AND METHODS: In 2021, based on archival data for 20192020 an assessment was made of the conduct of socio-medical rehabilitation at the State Budgetary Institution Center for Comprehensive Rehabilitation of the Disabled "Butovo". The observation unit is represented by stationary patient charts. RESULTS: The study revealed a sufficient volume and correct methodology for prescribing physical factors to patients with diseases of the nervous system, mental disorders, congenital anomalies and developmental delays. At the same time, the absence of an adequate goal of rehabilitation/habilitation was noted. The goals that were set for patients were often unrealizable, non-specific, not defined in time, or not related to the needs of the patient himself. CONCLUSION: A unified comprehensive approach is needed to assess the condition, identify needs and implement rehabilitation measures for patients with diseases of the nervous system, mental disorders, congenital anomalies and developmental delays. It has been determined that in this category of patients it is advisable to apply the International Classification of Functioning, which will allow one to concentrate on the necessary areas of diagnostics, select the necessary diagnostic tools, identify and link functional disorders and disability to each other, determine the areas of intervention of different specialists, unite specialists into one team with overall goal of rehabilitation/habilitation.
