Este estudo avalia a compreensão sobre o modelo de atenção básica nas ações de saúde. Trata-se de uma pesquisa de natureza qualitativa exploratória, desenvolvida em Unidades de Saúde da Família (USFs) de Marília, com amostra composta por profissionais de saúde, usuários e estudantes de Medicina e Enfermagem inseridos nas unidades. Os dados foram coletados por meio de entrevistas semiestruturadas e analisados pela técnica de análise de conteúdo, modalidade temática. Pôde-se observar que, independentemente da categoria entrevistada e da área onde se encontra a USF, a compreensão dos entrevistados sobre o modelo de atenção é o modelo médico-assistencial privatista, que reflete o predomínio das ações curativas em uma relação individualista, operada diretamente pelo médico e oferecida na demanda espontânea, não sendo exclusiva do setor privado. Foi proposto utilizar o espaço de grupos organizados para educação em saúde, em que trabalhadores e usuários são participantes para uma interação com as necessidades da comunidade e participação ativa dos usuários no planejamento do serviço, além de investimento nas ações de promoção e prevenção, o que causaria adesão da população e geraria credibilidade no modelo não médico-centrado.
Abstract. Introduction: tuberculosis (TB) remains a major cause of morbidity and mortality worldwide. The incidence of TB has increased since the 1980s. Given the increasing prevalence of TB worldwide, osteoarticular TB (OATB) is a significant health problem. Methods: retrospective study of a case series of hospitalized patients with confirmed OATB by culture or histopathological examination who were seen at a reference orthopedic hospital in São Paulo, Brazil, from 2014 to 2019. Results: thirty patients with confirmed bone and joint TB were seen from 2014 to 2019. The main sites of OATB were the spine (83.3 %) and the appendicular skeleton (26.7 %). Indication of surgical treatment was significantly related to the need for hospitalization (p=0.009) and the increased length of hospital stay (p=0.005). Presence of sequelae at the end of treatment was correlated with the presence of motor deficit at the time of OATB diagnosis (p=0.035) as well as with initial presence of functional limitation (p=0.025) and with high value of C-reactive protein at the end of treatment (p=0.037). Conclusions: the delay in the onset of clinical and laboratory signs of cases of osteoarticular infections hinders the early diagnosis and treatment of the disease, resulting in major complications sometimes requiring surgical treatment and consequently leading to a prolonged hospital stay, evidence of high inflammatory activities, and the presence of neurological deficits.
Introduction: The conventional method for measuring the ankle-brachial index (ABI) requires a vascular Doppler machine and a trained professional, which is a barrier to the examination becoming more widely adopted across health services. For this reason, the possibility of substituting Doppler monitors for other types of device has been investigated. The objective of this study was to assess the validity, reliability and accuracy of taking ABI measurement using oscillometric devices and compare them to vascular Doppler. Methods: This is an integrative literature review of four articles. Results: There was very little uniformity between the four studies in terms of ample populations or the methodological procedures used to measure systolic pressures. The results for sensitivity, specificity and positive and negative predictive values varied and so did measures of reliability. Conclusions: The results of these studies do not provide a basis from which conclusions can be drawn on the validity, reliability or accuracy of employing oscillometric devices as a substitute for Doppler for determination of ABI.
Objectives: To characterize the clinical-epidemiological profile of children with heart diseases (congenital, acquired and arrhythmias), conducted by the pediatric cardio group of a private general hospital, from January 2016 to March 2020. Method: Retrospective longitudinal observational interference study, with quantitative design, carried out in a private hospital. A total of 502 patients were selected according to the following inclusion criteria: patients who had congenital heart disease, acquired heart disease and arrhythmia, all managed by the pediatric cardiomyopathy group. Descriptive analysis of absolute and relative frequencies were represented through tables and graphs. Results: Of the 502 patients included in the study, 86.5% had congenital heart disease, 7.7% acquired heart disease and 5.8% arrhythmias. 50.2% were female, 49.6% male and 0.2% were not defined so far. Prematurity occurred in 19.3% of cases, presence of syndromes in 31.2 % and associated comorbidities in 34.7%. In total, 530 procedures were performed, of which 77% were surgeries, 18.5% hemodynamic procedures and 4.5% hybrid procedures. A risk score called RACHS-1 (adjusted risk for surgery and congenital heart disease) was used to categorize the surgeries. There was a predominance of category 3 (38.4%), followed by category 2 (28.6%), category 1 (18.2%), category 4 (9.6%) and category 6 (5.2%). Conclusions: The complexity of the profile of the found heart diseases reiterates that measures are needed for an integrated approach of different specialties and professionals for the treatment of these patients.
